Breast Enlargement

A Note on Breast Implants

Breast enlargement is performed using either saline or silicone gel breast implants. At Cosmetic Plastic Surgery Clinic, our patients receive detailed information about both implant types so that they can make an informed choice.

  • Silicone gel implants are generally placed above muscle because their feel and consistency is so similar to breast tissue. Because silicone implants come pre-filled from the manufacturer, the incision required is slightly larger than for a saline implant, and the preferred placement method is through the areola itself (the dark area that surrounds the nipple). This implant type is more expensive than saline and has about a 25% chance of encapsulation (abnormal firmness of breast).
  • Saline implants are nearly always placed beneath muscle because they do need the camouflage of the muscle structure. This implant type can be inserted through a very small incision (less than 1 inch), which is generally located within the breast fold (where the breast meets the chest wall).This implant has less than 1% chance of encapsulation and gives the breast a "perky" appearance that many women desire.

Determining Implant Size

Careful measurements will be taken during your consultation. Based on those measurements and your desired size, Dr. Castillo will provide professional sizer implants for you to try on. These breast enhancements sizers are available in both our Savoy (Champaign-Urbana area) and Bloomington offices. Our specially trained staff members are available to personally assist you and answer any questions you may have during the try-on process. This option offers a realistic visual comparison to help you get a good idea of what your results will look like.

The Procedure

For women who choose silicone gel implants, their implants will be inserted through an incision around the nipple, which becomes nearly undetectable within a few weeks. Most saline-filled implants can be inserted through a variety of incisions. At Cosmetic Plastic Surgery Clinic, Dr. Castillo has developed a procedure where an incision of less than 1-inch in total length is created in the lateral portion of the breast fold to insert the implant. Through this very small incision, access is gained to the space behind the pectoral muscle and the implant is inserted. The augmentation can be done if needed through an endoscope. This allows for a very small incision that heals quite well, a route that creates minimal inflammation of the tissue, and because all of the surgical procedure is done behind the pectoral muscle, the breast tissue is left totally undisturbed. Breast augmentation surgery is performed under general anesthesia on an outpatient basis.

Breast augmentation can be combined with other surgical procedures, including a breast lift for a comprehensive makeover to breast size and shape, as well as body contouring procedures like liposuction and abdominoplasty (tummy tuck).

After Surgery

Postoperative pain is generally well controlled with prescription medicine. If needed, we can use continuous infusion of a local anesthetic solution through a pump postoperatively (local anesthetic is slowly and continuously pumped into the breast pocket through a very thin tube) to reduce pain further.

Although heavy exercising (like jogging) is not allowed for three weeks, routine activities can be resumed one to two days following surgery. A specially designed bra provided by our office needs to be worn for 7 days. We utilize prophylactic antibiotics prior to and immediately after the surgery.

Dr. Castillo will provide a full description of the complications that can occur following breast augmentation surgery. Keep in mind that all breast implants are not considered lifetime devices, so it is possible that they may need to be replaced in the future if needed. While no one knows the life expectancy of an individual implant, it is likely that these may be quite durable devices (may last 20 to 40+ years).
