Q:  I am considering breast augmentation. How do I know which size implant I should chose?

A:  Making the decision about breast size is a very personal decision. If you have not already had a consultation with a reputable cosmetic plastic surgeon, I recommend that you talk with one. Your surgeon and his staff should be able to answer many of your questions about implants and sizes. They should be able to talk with you more about how to select the right sizing with your particular body shape while keeping your goals in mind. Here are 3 options that can help you make a visual decision about sizing.
OPTION #1:
The majority of surgeons use “the baggie of rice method,” although Dr. Castillo offers a much more realistic option for his patients in the Savoy (Champaign – Urbana) office. To utilize the at home baggie of rice method, you can put rice, oatmeal, or birdseed in a sandwich baggie. Do not use a zip lock baggie as they are too firm. (1 ? cups = 10 ounces or 1 ? cups = 12 ounces…) Most breast augmentations utilize between 9 – 14 ounces for each breast (270cc – 420cc per breast). You should probably start with 10 ounces then adjust as you prefer. Select a bra for the size you want to be. Put the filled baggies inside the unpadded bra. Once you find the size baggie you are comfortable with, you may want to wear it for a few hours, with different shirts, do some normal daily activities…this will help you decide if the size is truly comfortable.
OPTION #2:
In our Savoy office, Dr. Castillo provides the option of trying on professional sizer implants in the office. This option by far offers the most natural, realistic visual comparison.  We have a variety of sizes so that our patients can get a good idea of what their results will look like.
At the time of the consultation, we give the patient an unpadded bra. The patient can also bring one if she knows about what size she would like to be. I suggest that our patients bring 2 different style tops; one form-fitting, one loose fitting. This gives our patients a great feel for how each size can change their body appearance. Of course our specially training staff is available to personally assist each patient and answer any questions they may have. Our patients love this option.  I think they feel much more confident in their decision.

I have viewed the DVD. I think it would be helpful for anyone who is considering breast implants. The DVD introduces you to a patient who has Natrelle breast implants as she shares her experience with her daughter who is now considering breast enhancement. You will view some before and after photos of breast augmentation. More importantly, the DVD takes you through a consultation so you know what to expect. I think this kit will be helpful in preparing you to ask the right questions when you are talking with your surgeon.

I would recommend using your own unpadded bra with the sample implants. The sample implants from this kit allow you to try them on with different pieces of clothing in the privacy of your own home. This will help you get a good idea of what size implants you will be comfortable with. In my opinion, they do not give near the realistic visual as the professional implants that we have in the office. The implant samples from the kit do not provide as smooth or realistic shape as our profession implant provide.  Therefore, even if you purchase the kit, I would still recommend taking advantage of the profession sizer fitting at our office.

Mommy Make-Over

As little girls, we’ve all watched enough Disney to know that, just in the nick of time, our fairy god mother arrives, waves her glamour wand and poof – happily ever after begins.
Then we have our first child and reality sets in. Who knew such beautiful little angels would do such unspeakable things to our bodies! If you have ever thought “Where is my Fairy Godmother now?” you’re not alone. I think most of us have stood in front of a mirror months or even years after giving birth in hopes that Fairy Godmother will arrive and grant our wish for a mommy make-over! Just like Cinderelli, we take care of everyone around us. We “make the fire, fix the breakfast, wash the dishes, and do the mopping…”
As moms we give so much of our time, energy and love to everyone around us that we often forget to take care of ourselves. We’ve all heard the term “kids can be cruel.”  We just didn’t know it was in reference to what was about to happen to our bodies after pregnancy! We are left with sagging skin where our waist used to be. Breasts are now distorted and stretch out. There is weight gain of the hips and thighs. Our new saddlebags become a quick reminder that we are probably no longer eligible for low rise jeans.
For most of us, pregnancy takes a huge toll on our body. With child-bearing, raising a family, car pool, homework, household chores, not to mention sun damage, age and heredity hovering over us, our own well being and self development can get lost in the shuffle. We are so busy that we don’t even notice until someday, a friend or even a complete stranger makes a comment that really sticks.
Perception is reality. How we view ourselves and how we think others view us determines much of how interactive we are with those around us.  All of the changes we see on the outside can have some damaging effects on the inside as well. We lose self-respect, confidence and spirit.  We become less socially active. Sound familiar?
Since you are reading this blog, I am going to assume there is something about your body that makes you uncomfortable. What is it that bothers you? Do you find yourself repeatedly thinking about a particular area of your face or body?  How many times have you thought about it today? In the last 6 hours?
Maybe it’s your abdomen. Maybe it just hasn’t recovered after the birth of your second child. Often, it doesn’t matter how health conscious and physically fit you are. You cannot exercise away the stretched out, saggy skin hanging from your abdomen that once provided a safe place for your precious bundles of joy to develop. Maybe you find yourself so self-conscious that it is becoming more and more difficult for you to expose your body to your partner. You’ve traded in your bikini for a one piece with a skirt. You find yourself selecting styles of clothing that attempt to hide your body instead of ones that say you are vibrant, assured women.
Maybe you are a single mom who has a strong desire to begin the search for your prince charming. But, saying yes to that first date has you terrified. You don’t feel good about the changes that have occurred in your body. That stops you from coming across as the fun, outgoing, intellectual that you are. Maybe it’s the uncontrollable, extended greeting your arm gives; you know, when the underside of your arm continues to wave to your friend long after you’ve stopped. Maybe you’re an empty nester now and you would like to finally do something for yourself.
Many women are no longer waiting for their fairy godmother to wave her magic wand. They are taking charge, no longer accepting that their distorted bodies are the price they have to pay for having a family. I guess you could say that they are asking Dr. Castillo to wave his magic scalpel instead.
Often, Mothers walk into our office for the first time with their shoulders hanging low. They are reserved, tired, insecure. By the day of their surgery, a change has already begun. It takes courage to take charge and force a change. Of course they are a little nervous. But they are so excited! How many people do you know who head off to surgery with a big smile on their face?  After their “mommy make-over,” they become renewed. They are suddenly reunited with the person they used to be.  Their families have miraculously survived during the recovery period. They have a new brand new attitude. They are energetic. They laugh more. They smile more. They walk straight up, looking out at what the world has to offer.
Cosmetic surgery is not the right choice for everyone. If you are considering it, do your homework. Ask lots of questions at your consultation. If you do not feel comfortable during the consultation, seek out other doctor. You need to develop realistic goals. Having cosmetic surgery will not fix your problems. However, substantial changes can occur in patients who choose cosmetic surgery for the right reasons. It often changes their quality of life. In many cases, patients become motivated to live a more health conscious, socially active lifestyle.
As mothers it’s important to remember that we are also individuals. It’s okay to be a mom and still want to be sexy, current and confident. In fact, who could possibly be a better role model for your children than a self-assured, strong, healthy, enthusiastic mommy?

Q: Lately, friends have made comments about me looking tired. They are right, I do look tired. My skin just looks dull. I am starting to develop lines and brown spots! I even seem to have extra “baggage” around my jaw line. I am only 43 years old! How can that be? I am very a bubbly, energetic decorator / designer who sleeps and eats well. I haven’t really worried about skincare in the past because my skin has always looked great. How can I avoid looking older than I am and definitely older than I feel? Is it too early to start using anti-aging products and treatments?

A: I think the key to longevity is early maintenance. You mentioned you are a decorator, so bear with me as I “attempt” to speak your language…
Think of your skin as a home. Your home can be a beautiful older home or it can be an old home. The difference between a beautiful older home and an old home is maintenance. A well maintained home that is freshly painted and properly cared for with necessary maintenance becomes a beautiful older home.  Otherwise, if we neglect our home, the only thing we’ll have is an old home. We must do those necessary repairs as they are needed, or one day you may wake up to find out that your once beautiful home is worn down and no longer reflects who you are anymore. By this point, it may need to be completely remodeled before you feel at home in it again (surgery).
There are three things that you absolutely need to do to your skin to maintain it.

Think of your skin as a home. Your home can be a beautiful older home or it can just be an old home. The difference between a beautiful older home and an old home is maintenance. A well maintained home that is freshly painted and properly cared for with necessary maintenance becomes a beautiful older home. Otherwise, if we neglect our home, the only thing we’ll have is an old home. We must do those necessary repairs as they are needed, or one day you may wake up to find out that your once beautiful home is worn down and no longer reflects who you are anymore. By this point, it may need to be completely remodeled before you feel at home in it again (surgery).

There are three things that you absolutely need to do to your skin to maintain it.

  1. Daily (that’s 365) use of UVA / UVB sunscreen with SPF 45 or higher.
  2. Continued exfoliation (retinol, Retin-A, glycolic acid, microdermabrasion, chemical peels…)
  3. The proper use of moisturizers.

The best way to avoid looking older is to protect your skin 365 days of the year. Many patient’s have said “Roxanne, if I only use one skincare product on my skin, what would you recommend?”  I can assure you my answer is always the same – “UVA / UVB broad- spectrum SUNSCREEN.”  All other anti-aging skincare products are pointless if you are not using sun protection.  I would recommend that you refer back to two earlier blog posting’s (if you haven’t already).

Medical-grade skincare products are the best way to properly moisturize and maintain the skin. These products are dispensed only by a physician. They are not available over-the-counter because they are generally of a much higher concentration. A daily medical-grade skincare program will help maintain results from skin treatments and reverse signs of aging.

There are some simple, effective, in-office treatment options for early maintenance. Medical microdermabrasion, laser treatments, chemical peels, or intense pulsed light photo facials can help to restore a youthful appearance. You might be surprised how much younger one can look by just evening out the skin tone, removing brown spots and keeping the skin well hydrated. Q: I have a drawer full of lotions and creams that don’t work. There are so many skincare products out there.  How I know which skin care products work and which ones do not?

A: Well, first of all, you are on the right track. Maintaining youthful, healthy looking skin does require an effective home-care routine. With the abundance of the over-the-counter and infomercial skin care products available today, I understand how confusing and frustrating it can be to choose effective skincare products without wasting your time and your money.
My first recommendation:
Consult a reputable doctor who specializes in facial aging such as a cosmetic or facial plastic surgeon. Remember, just because “M.D.” appears behind their name, it does not make them an expert in every field of medicine. Technology is always changing, so it’s important to consult with someone who is up to date on those changes. Many times cosmetic or facial plastic surgeons will have a Medical Skincare Specialist on staff, such as myself.
My job is to take the guess work out of the equation. Most often, patients come to the consultation, very frustrated. They just want to look their best. But they feel like they have wasted a lot of time and money experimenting with different products, just trying to narrow down what works and what doesn’t.
During a skincare consultation at Cosmetic Plastic Surgery Skin Restoration and Laser Institute, we listen to the patient’s skin concerns and their goals. Then we talk about the options they may have.  During this time you get lots of great, FREE information to help them understand what ages the skin, how to avoid it, and works for their particular skin issues. They get a better understanding of what each product does and how to best maintain results. We de-bunk many of the media driven myths that are out there and answer any questions the patient may have. I would also recommend that you do not relay on magazines, talk shows, or infomercials as much of that information is misleading.

So, I guess we shouldn’t be surprised when it doesn’t work right?
I cannot stress enough the difference that a medical grade skincare program can make in the way your skin looks, acts and feels for the same cost as you would pay at the mall. The main differences between over–the-counter-skincare and medical-grade skincare are strength and effectiveness.  Medical-grade skincare is typically formulated at a much higher strength, as it is used under the care of a physician. Medical-grade skincare is tested in a blind or double-blind clinical study to prove its effectiveness. Over-the-counter skincare is not. Therefore you do not know if you are really getting what you are paying for. Unlike over-the-counter skincare, medical-grade skincare must be FDA-approved. The ingredients are listed in order of quantity as well.

Real or Fake… You Decide

We know that for decades long, full lashes have been a sign of higher beauty. So what makes those who tout lengthy, charming lashes leave a lasting imprint on the minds of so many? Think about it. If you were asked to name some of Hollywood’s leading ladies from the past, who would come to mind…Doris Day, Audrey Hepburn, Elizabeth Taylor, Marilyn Monroe. What do you remember about their beauty? It’s their eyes right? These American Beauties were seen all around, flaunting their graceful yet alluring lashes on the silver screen, in photos and magazines. It made them appear mysterious, desirable, and memorable.  What woman doesn’t want that? Sign me up!
