Possible Rosacea Triggers

Though there is no cure for Rosacea, I hope the following lists, compiled from patient histories by Dr. Jonathan Wilkin and produced by the National Rosacea Society will make it easier for you to determine your own triggers and avoid them.

Foods:
Liver, Yogurt, Sour cream, Cheese (except cottage cheese), Chocolate, Vanilla, Soy sauce, food-eating-candy-chocolate
Yeast extract (bread is okay), Vinegar, Eggplant, Avocados, Spinach, Broad-leaf beans and pods (including: lima, navy or pea). Citrus fruits (including: tomatoes, bananas, red plums, raisins or figs), Spicy and thermally hot foods, and Foods high in histamine.

 

Beverages:

The Liquid Facelift – Truths In Reporting and Advertising?

On August 12, 2009, CBS had a program in which they demonstrated the so-called “liquid facelift.”  As it is shown, there were injections placed in appropriate areas of the face to diminish facial lines and furrows by injecting a product called Sculptra TM.  It incorrectly refers to Sculptra TM as being a  filler that was just recently approved, since it has been approved by the FDA for years for the treatment of immune-suppressed patients, and particularly directed at AIDS patients.
I take issue with describing this injectable as a “liquid facelift.”  In fact, it is not.  It is simply an injection to replace some of the volume loss in certain areas of the face.  It definitely does not give or even approximate the results of a mini facelift or regular facelift.  To compare the results of an injectable to a facelift is grossly unfair to the public and plays on the sensitivities of those who are looking for an alternative to surgery.  If what an individual needs is a facelift, then they are wasting their money on temporary filler.
Please do not misunderstand me.  There is nothing wrong with injectables.  I use them in my practice frequently with great success.  The whole idea is that it is very important to match the problem with the correct solution.  By that I mean that if there are already set-in facial problems like jowls, sagginess and volume loss, the most effective way of dealing with it is through a surgical procedure. Anything else would be at best a compromise.  For most patients, it would be a waste and a disappointment.
If, on the other hand, the patient is looking to treat early aging changes such as volume loss without sagginess or jowls, commonly seen after age 30, then an injectable such as Sculptra TM would be a good alternative. It will help minimize some of the volume loss effects of aging for approximately a year or two.  However, the physician injecting this filler needs to have a very good artistic eye in order to be able to inject the filler where it will have the most effect.  The goal is to minimize the normal shadows of light and to minimize the highs and lows that tend to develop as a result of facial aging.
The cost is another factor one needs to consider since it is likely that one may need $1,000.00 and $2,000.00 worth of Sculptra TM to accomplish the desired effect.  This needs to be taken into consideration because when one uses facial filler the effects are only temporary.  Within a period of 6 to 24 months, the effect is completely lost. When comparing this with the results of a surgical procedure like a facelift, one must realize that though the effects of a facelift may be modified by time (the aging process), the improvements will be with the person for life.
The report stated that you could go out the night of the injection and that is in essence, untrue.  Even in the hands of the most skilled professional, a moderate amount of swelling occurs with the injection of any filler within the face.  Bruising can also occur.  One should probably allow 3-4 days after an injectable before planning to attend a social engagement.
As with any other filler, Sculptra TM is a volume replacement agent which will be useful on a temporary basis. We should never call an injectable a “liquid facelift.”  We should call it exactly what is it, which is the correction of volume loss by use of a facial filler (injectable). THE GEL –

Ask Our Expert: Rosacea

Q: I have had Rosacea for several years. It seems to be getting worse. It has become embarrassing during social situations. I have tried a few creams recommended by my doctor. I didn’t see much improvement.  Can you recommend other options to try?

A:  Rosacea can be difficult to live with and tricky to treat. There is no cure for Rosacea. We can only treat the symptoms to make it easier to control. It generally requires more than one type of treatment to combat the symptoms. There are still many medical unknowns related to Rosacea. However, over the past few years, we have gained a much better understanding of how to reduce the symptoms of Rosacea.
My first recommendation is that you keep a journal for awhile to help you indentify the things that trigger your Rosacea symptoms. Once identified, try to reduce the exposure you have to your triggers.
Some triggers may include (but are not limited to) the following:

A:  Rosacea can be difficult to live with and tricky to treat. There is no cure for Rosacea. We can only treat the symptoms to make it easier to control. It generally requires more than one type of treatment to combat the symptoms. There are still many medical unknowns related to Rosacea. However, over the past few years, we have gained a much better understanding of how to reduce the symptoms of Rosacea.

My first recommendation is that you keep a journal for awhile to help you indentify the things that trigger your Rosacea symptoms. Once identified, try to reduce the exposure you have to your triggers.

Some triggers may include (but are not limited to) the following:

  1. Sun or wind exposure
  2. Cold or hot temperatures
  3. Spicy foods
  4. Alcohol consumption
  5. Stress
  6. Hot drinks

The proper treatment selection will depend on the symptoms you are experiencing.  Papules or pustules are often treated with antibiotics such as erythromycin or tetracycline.  Topical treatments (medicated creams…) can also be used in conjunction with oral medications. The topical treatments can also be used to reduce the burning sensation that is sometimes related to Rosacea.  Redness may be reduced over time as well.  If papules or pustules are present, low-concentration medical-grade glycolic acid peels may also be used in conjunction with the above mentioned treatments. This will help to thin the most outer layer of skin which may help to resolve some of the eruptions and can also encourage better penetration of the topical medications being used.

Telangiectacia (superficial broken vessels) is a universal component of Rosacea. It will not respond to the above treatments.   A uniform treatment of  light-therapy (Intense Pulsed light or laser) will improve telangiectacia and redness.  Not all Intense Pulsed Light, laser systems or operators are equal.  010It is important to select an expert to plan your treatment program.  Remember experience is the key to getting the results you want. Expect to have a series of light treatments, approximately 3-6 treatments about three weeks apart. Once the series is completed, the redness and telangiectacia should be much improved. Over time, it is likely that “touch –up” treatments will be needed to keep it under control.

I have not been that impressed with topical agents for redness in the past.  However, about 1 – 1-1/2 years ago, we began using a new medical grade Rosacea cream.  Within a few weeks our patients were reporting that they could see a reduction in facial redness. Our patients come back to the office to purchase more of this product BEFORE they run out of it.  That tells me they are continuing to have long term success with this product. This product is an excellent choice for post light therapy maintenance.

Keep in mind that while exfoliation is still important, you should choose methods that do not include using abrasives or scrubs on your skin. Controlling Rosacea requires persistently protecting your skin from sun exposure.  Using UVA / UVB broad-spectrum sunscreen 365 days per year is of great importance.  I would suggest a medical grade physical sun block containing high percentages of Titanium Dioxide or Zinc Oxide.  Wear sun glasses and a wide-brim UV protected sun hat when you are outside for additional protection.

Q:  I am considering breast augmentation. How do I know which size implant I should chose?

A:  Making the decision about breast size is a very personal decision. If you have not already had a consultation with a reputable cosmetic plastic surgeon, I recommend that you talk with one. Your surgeon and his staff should be able to answer many of your questions about implants and sizes. They should be able to talk with you more about how to select the right sizing with your particular body shape while keeping your goals in mind. Here are 3 options that can help you make a visual decision about sizing.
OPTION #1:
The majority of surgeons use “the baggie of rice method,” although Dr. Castillo offers a much more realistic option for his patients in both the Savoy (Champaign – Urbana) and Bloomington offices. To utilize the at home baggie of rice method, you can put rice, oatmeal, or birdseed in a sandwich baggie. Do not use a zip lock baggie as they are too firm. (1 ? cups = 10 ounces or 1 ? cups = 12 ounces…) Most breast augmentations utilize between 9 – 14 ounces for each breast (270cc – 420cc per breast). You should probably start with 10 ounces then adjust as you prefer. Select a bra for the size you want to be. Put the filled baggies inside the unpadded bra. Once you find the size baggie you are comfortable with, you may want to wear it for a few hours, with different shirts, do some normal daily activities…this will help you decide if the size is truly comfortable.

I have viewed the DVD. I think it would be helpful for anyone who is considering breast implants. The DVD introduces you to a patient who has Natrelle breast implants as she shares her experience with her daughter who is now considering breast enhancement. You will view some before and after photos of breast augmentation. More importantly, the DVD takes you through a consultation so you know what to expect. I think this kit will be helpful in preparing you to ask the right questions when you are talking with your surgeon.

I would recommend using your own unpadded bra with the sample implants. The sample implants from this kit allow you to try them on with different pieces of clothing in the privacy of your own home. This will help you get a good idea of what size implants you will be comfortable with. In my opinion, they do not give near the realistic visual as the professional implants that we have in the office. The implant samples from the kit do not provide as smooth or realistic shape as our profession implant provide.  Therefore, even if you purchase the kit, I would still recommend taking advantage of the profession sizer fitting at our office.
