Ask Our Expert – Outpatient Surgical Facility Vs. Hospital

Question: Why do you prefer to perform surgery in an outpatient facility rather than the hospital?

Dr. Castillo’s state-of-the-art outpatient surgical facility.

Answer – Dr. Castillo: Actually, about 90 percent of cosmetic plastic surgical procedures are now done in outpatient surgical facilities for the following reasons:

 

Safety

The surgical procedures done in an outpatient surgical facility are done by a staff which is dedicated to doing only outpatient surgical procedures, so they are better focused, the staff is better experienced and dedicated to a single task.  This increases safety and provides the patient with knowledgeable people that are experienced in state-of-the-art outpatient surgical procedures.

*    Infection

The rate of infections in hospitals varies anywhere between 2 to 12 percent.  The rate of infection in outpatient surgical facilities is much smaller.  In fact, in our outpatient surgical facility, infection is a nearly non-existent problem and we say that because it only presents itself every few years.  That is, the rate is so small that it is difficult to quantify.  You may ask yourself, why?  The reasons are multiple but for the most part it has to do with carrying out procedures that are done in patients who are healthy and not suffering from disease and/or infection.  Also remember that in our facility, we do not do what is referred to as a “dirty case,” that is, procedures that involve entering the intestine, colon or areas that typically have bacteria.

*     Cost

The cost of care in a hospital facility is often unknown and extremely high.  The reasons have to do with hospitals having fees that are much higher than the real expenses to be able to cover the uninsured and establish artificially high rates that can then be applied to different insurances.  The reason why this is possible is that generally insurance have no choice but to pay a fee while in our facility, the choice is yours.  That is, you are the patient and you elect whether to pay or not pay the cost you know beforehand to be – that is, you approve of that cost.  What this results in is a great competitiveness and a much reduced cost for the surgical procedure.

*        Scale

When you go to a hospital, you immediately become part of literally hundreds or even thousands of people who may be at the hospital for one reason or another.  That makes you one of many and the services are going to be diluted, meaning you are not likely to have individual care.  In comparison, an outpatient surgical facility like ours has staff that is dedicated to only you and no other business to take care of during the time that you are at the facility.  Consequently, you have a staff member that remains with you throughout your surgical procedure.  There are no delays and this results in expedient care and a much reduced level of anxiety.

*     Comparative Services

Are the outpatient surgical facilities like ours able to take care of you in an emergency?  The answer is yes.  Our facility is equipped to respond to an emergency in the same manner as a hospital would be.  That is, our facility is accredited by The Joint Commission https://www.cosmeticplasticsurgery.com/cosmetic-plastic-surgery/accreditation.cfm, which is the accrediting body for most hospitals in the nation.  This means that the level of response, equipment, drugs and supplies is par with a hospital.  What the outpatient surgery facility has that the hospital does not, is the experience to deal with the type of surgical procedure, that is, cosmetic surgery, which you are going to have.  This is an area, like in most areas, where expertise is not only important but is often the determinant for the degree of satisfaction and success that the procedure has.

So regarding the question, are you better off having your surgical procedure in a dedicated outpatient surgical facility doing cosmetic surgery only?  The answer is a qualified yes for the above reasons.

Mommy Make-Overs For Empty Nesters

I had dinner with one of my dearest girlfriends recently.  By the start of our second glass of wine we got right down to the nitty gritty – “L.A.C. of purpose” (Life After Children).  She is now an empty nester and life without her children in her home has been a serious adjustment  as life as she has known it, is now changed forever. She is a bright, beautiful, enthusiastic woman who runs a multimillion dollar company yet she found she is lacking something…a passion or purpose in this new empty nesting stage of her life. She isn’t sure how to fill all of the evening time that used to be absorbed by the kid’s activities and shared that home just doesn’t feel the same.  The idea of focusing on herself is new territory and the process is a slow one. She has been the ultimate caregiving for years. It is what she does, part of her identity.  I have spoken to many of my mom friends who have also been deflated by this life changing event. It is indeed a lot to take in.

As I have one child down and one to go, I am listening to them carefully and taking notes. I’m preparing myself for that same empty nester syndrome as we speak. I have been told that I was a little crazy for about a year before my oldest daughter left me behind to further her education.  As the days rush closer and closer to senior year for my youngest, I can feel a pinch of sadness each morning as I flash her a smile and wish her a good day at school, knowing there are only 191 more opportunities to say that ever. I’m already starting to morn her absence and she hasn’t even left yet!

It seems to me that no matter how we try, moms just can’t emotionally prepare for this event, we just can’t. It just leaves a hole in the middle of our chest. And there is no one who can fill it but us. As little girls we prepare for adult life by pretending to get married on the school playground and spending countless hours mothering our dollies.  But in all those years, I don’t ever remember pretending that the kids actually grew up and left home!  My dolls never got too old to sit on my lap and listen to their favorite bedtime story or too old to be seen being dropped off at the movies by their mommy.  I don’t ever remember walking into the red, white and blue playhouse my dad built for me, pretending to return home from work to be over- come by the sadness my homes new silence brings when the kids aren’t there to greet me… no more banquets or ballgames to rush off to…just a perfectly clean, perfectly silent, perfectly empty, nest.

Being a mom is like a continual series of small demotions until one day when we finally get fired or retired – I’m not really sure which it is…

As a mom you have dedicated your life to attending ballet recitals, baseball games, band concerts, weekend volleyball tournaments, school plays and talent shows (where of course your child was the best, oh wait, no, that was mine  lol).  You have played Pokémon, Barbie’s, and settled fights over who gets to be the RED Power Ranger. You taught your child to ride a bike and master the multiplication tables.  You have watched endless hours of Bugs Bunny, Banana’s In Pajamas, and Boy Meets World.  You know you’ve caught yourself singing Barney songs in the car even when you kids weren’t with you!  You’ve kissed boo boos, dried the tears of a bad haircut and had uncomfortable but necessary sex talks. You’ve cured everything from colds to crushed hearts.  You’ve dragged your entire family’s luggage from one side of the airport to another all because your little princess wanted a Cinnibon. You have thrown school holiday parties, picked out prom dresses and first cars, spent hundreds of hours worrying about facebook, dating, drugs, alcohol, and college degrees.

You have planned carefully to teach them all of the lessons they would need to successfully go out into the world as a responsible, respectful citizen and then at some point they actually have the audacity put those tools work.  They have left your nest.  Now what?

Now what?  That is the question many empty nesters are asking. Well you can spend day after day curled up under the covers of their empty bed looking at photographs from years past or you can decide that it’s finally time to focus on you and find your next adventure in life.

I see this same empty nester syndrome in many of our patients at Cosmetic Plastic Surgery Clinic.  The ones I see are the take charge empty nesters who aren’t sitting back in their rocking chairs though.  They have been through the dark side of empty nesting and came out swinging.  That is the path I hope to take.  They are finding new careers, new passions, new hobbies. They are taking time to eat better, exercise more, and try new things.  They are acknowledging and take care of the issues that bother them and embracing the things they enjoy.

It takes a toll on both your body and your spirit.  Too many nights sitting up polishing off the last of the Ben & Jerry’s while waiting for your new driver to arrive home safely has earned you a thick mid-section to match the already present muffin top, sagging skin and weak abdominal muscles you earned with the birthing of your children. You have lived far too long with elongated, deflated sacks that you used to recognize as breasts before breast feeding your little darlings.

It seems like society has placed mothers in a role that is much like a super-hero of sorts.  More than in years past, today’s moms are BUSY! We do it all! We are educated, over-scheduled, over-worked, under-rested creatures who find time to affectionately care for everyone but ourselves. I often here from mommy make-over candidates that they have been bothered by the appearance of their post-pregnancy breasts and body for years but just never had the time or left over energy to do something about it while they were still raising children.

Samuel Ullman once said “Years may wrinkle the skin, but to give up on enthusiasm wrinkles the soul.   So this Mother’s Day wish for all the empty-nest mommy’s out there is to take this opportunity to give yourself a mommy makeover, whatever that may be to you.  You have done your job.  You have introduced kind, successful, respectful citizens into society.  Take a breath, pat yourself on the back.  Now focus on you! Make a list of things you’ve always wanted to do and get started!  Discover new books, old passions, take better care of your health and your skin, take an adventure with a good friend, or maybe even get your body back with a Mommy Make-Over by Dr. Castillo.  You have certainly earned it!  With any luck you will re-discover yourself and your adult children will discover that their confident, interesting, evolving mom is someone they really enjoy spending time with.

Have you have had that shocking moment where you stumble out of bed, make your way toward the shower, pass the mirror, catch a peek on the way and see…what…rub your eyes to get a more clear view, fumble for your spectacles, say a quick prayer, and head back for another look in the mirror… “what?  How can that be??? Chest wrinkles?  Chest wrinkles!  Really!  Oh come on, you’ve got to be kidding me.  This is not the best way to start a day.”

Score: Mirror – 1, Self-Esteem – 0.

Even those of us who have the greatest capability for denial somewhere down deep recognize that someday, far, far away of course, but some day, we will develop a few lines or wrinkles on our face…but on our chests??  Is this some kind of cruel joke?  “Is this nature’s way of punishing me for that time I didn’t return the shopping cart to the cart coral at Target because it was raining and I didn’t want my hair to turn into a frizz ball?”   Rest assured (though I know this is no real comfort) at some point most of us have a moment, maybe not a chest moment, but a moment where some part of the aging process knocks us off our feet for a bit.

The good news is science is prepared to re-inflate the bubble that nature just popped.  Bless the brilliant men and women who have dedicated their lives to creating and improving laser technology. There should be a holiday, maybe even two, named after them don’t you think?

The other good news is you are not sentenced to a life of hiding your wrinkled cleavage.  There are a few ways to correct the damaged skin of your chest.

Ask Our Expert: What is the best type of sunscreen?

Q: What is the best type of sunscreen?

A: Historically, it has been shown that physical sunscreens provide the best protection against both UVA & UVB radiation and are less likely to cause skin irritation.  Dr. Castillo, cosmetic surgery and anti-aging expert  https://www.cosmeticplasticsurgery.com/ recommends medical-grade, physical sunscreens to all his patients.

There are two divisions of sunscreen ingredients; physical and chemical. Physical sunscreens reflect the UV radiation while chemical sunscreens absorb the UV radiation and break it down within the skin. Some sunscreens contain both a physical and a chemical component.

There are two types of physical sunscreens available; zinc oxide and titanium dioxide. Both provide broad spectrum UVA and UVB protection. Today, these ingredients can be found in finely micronized, nano-sphere  form which allows for ease of daily use. Not only does zinc oxide and titanium dioxide provide needed broad-spectrum coverage but they are also beneficial to those who have sensitive skin or are irritated by chemical ingredients.

You will typically find multiple active ingredients listed in chemical sunscreen.  Each chemical ingredient protects only against a specific portion of the UV spectrum. Be aware that at this time many over-the-counter sunscreens protect against UVB rays only, although there are a few chemical ingredients like Oxybenzone that do offer UVA/UVB protection. Therefore, when purchasing over the counter sunscreen, it is important to examine the label for the claim “broad-spectrum.”  Also look for a sunscreen that says “very water resistant.”  However, keep in mind that even the best water resistant sunscreens can only protect you for 80 minutes.  Then you will need to re-apply.

With inconsistent OTC product labels and the idea that active ingredients in sunscreens differ from manufacturer to manufacturer it can be difficult to decipher all of them so I have provided at chart of the more common ingredients found in sunscreens.  When in doubt whether a sunscreen will effectively protect you and your family, I recommend selecting a medical grade sunscreen from your local cosmetic surgeon, plastic surgeon or dermatologist. Our patient’s favorite sunscreens are:

Sun Defense by Castillo MD Skin Science, which offers medical grade 12% Zinc Oxide, 7.5% Octinoxate broad-spectrum UVA/UVB protection

Say Goodbye to Chest Lines, Wrinkles and Brown Spots!

I was speaking with one of Dr. Castillo’s breast augmentation patients during a skin rejuvenation consultation.  She explained to me that a few years back she had breast enhancement https://www.cosmeticplasticsurgery.com/cosmetic-surgery/breast-enlargement.cfm in Dr. Castillo’s Cosmetic Surgery facility in Savoy, Illinois and that she couldn’t be happier with her results and the entire Cosmetic Plastic Surgery experience.

However, she has some fine lines and wrinkles on her chest between her breasts along with a moderate amount of brown spots. Castillo M.D. and Cosmetic Plastic Surgery Skin Restoration and Laser Institute offered skin treatments to remove the wrinkles and brown spots of the chest.

I explained to her that the vertical lines and brown spots she was describing are common among those who have been over-exposed to the sun. The sun breaks down elastin and collagen of the chest, creating lines, wrinkles, and sagging skin.  It also causes melanocytes (pigmented cells that create brown spots she is seeing on her chest) to over-populate the surface of the skin in attempt to protect itself from damaging UV radiation.

I explained to her that yes, we can help with her chest lines and brown spots.  To improve the look of her damaged skin, first, she would need to begin to protect her décolletage using medical grade broad-spectrum sunscreen 365 days per year.  UV radiation will create more severe damage to areas such as the chest, neck and hands. The characteristics of the skin of the chest, neck and hands simply cannot tolerate the same amount of UV radiation as the face. The skin of the chest is not as forgiving as skin surfaces such as the face when it is repeatedly over-exposed to the sun. The skin of the chest is thinner, containing less elastin / collagen.  As the elastin is broken down, lines, wrinkles and sagging of the skin WILL occur.

Unlike the face, skin of décolletage does not contain an abundance of oil glands.  Why is this a problem?  Having an abundance of oil glands is important because as oil glands cells divide, they can become almost any other type of epidermal cell.  Therefore, when cells get damaged, they provide new cells to help to restore your damaged skin.

The good news is there are a few ways to correct or at least improve the skin of your chest.

  1.  First, and most importantly, PROTECT your chest for the sun.  Use a medical-grade, broad-spectrum, physical sunscreen  every day of the year. I recommend one of two  physical sunscreens:  Sun Defense or Prime Defense By Castillo MD Skin Science.  Both have high percentages of physical block ingredients (Titanium Dioxide and / or Zinc Oxide) making them superior  choices for sun protection.
  2. Avoid tanning beds as UVA rays will only create more damage to your skin.
  3. Use UV protective clothing and a wide brimmed hat to provide additional protection.
  4. Consult G.D. Castillo M.D. or another Cosmetic Plastic Surgeon regarding the following treatment options:
    1. High Vegetable Protein Recipes – Enjoy!

 High Vegetable Protein Recipes

As a follow up to his “The secrets of permanent weight loss” blog, G.D. Castillo, M.D. would like to share a few of his favorite vegetable protein recipes to get you started. We, at Cosmetic Plastic Surgery Clinic hope you enjoy them as much as we have.

Lentils with Pork

(For vegetarian recipes omit entirely the pork loin).

Ingredients:

2 lbs. rinsed lentils (serves about 12 portions)

1 or more serrano or jalapeno peppers (omit if you do not like heat)

1 lb. of chopped pork loin

3 large tomatoes

¼ large onion, minced

3 wedges of garlic, minced

3 tbsp extra virgin olive oil

½ tsp. Spanish paprika

½ tsp. salt

Directions:

Using a 4 qt. pot, put olive oil and minced onion, garlic, diced peppers, spices and pork and sauté for approximately 8 minutes.  Add chopped tomato and continue to sauté for 5-8 minutes.  Add 2 quarts of water and lentils, bring to a boil and simmer for 1 and ½ hours.  The lentils will be done when they break open or are soft enough to break open with minimal pressure from your fingers.

Mediterranean Chick Peas (Garbanzo Beans)

Ingredients:

2 lbs. of garbanzo beans

8 thick slices of bacon (cooked)

4 medium tomatoes

1/3 of a large onion

3 garlic wedges

½ bunch of Italian parsley

1 or more jalapeno peppers (omit if you do not like heat)

3 tbsp. of extra virgin olive oil

½ tsp. of salt

Directions:

In a 4-qt. pot sauté chopped bacon until lightly brown and discard the bacon oil. Add olive oil, diced peppers, onion and garlic.  Sautee for approximately 5-8 minutes.  Add sliced tomatoes and sauté for another 8 minutes.  Add 2 qts. of water and rinsed chick peas.  Add ½ bunch of chopped Italian parsley and bring to a boil.  Simmer for approximately 2 hours. Add water if needed.  The consistency should be that of a heavy broth, so do not add excess water.

Black Beans

Ingredients:

2 lbs. of black beans, rinsed

¼ large onion

1 bunch of chopped cilantro

1 or more serrano or jalapeño peppers (omit if you don’t like heat)

3 tbsp. of extra virgin olive oil

¼ tsp. of salt

Directions:

In a 4 qt. pot, place olive oil, diced onion, peppers (if desired) and sauté until lightly brown.  Add 3 qts. of water and add black beans.  Add the chopped cilantro.  Cover with a lid and bring to a boil.  Simmer for 2 ½ to 3 hours.  You may need to add water as it simmers.  The beans will be done when they break open or are soft enough to break open with minimal pressure from your fingers.

Tips for preparing legumes (lentils, chick peas, beans…):

1.It is important to cook the legumes thoroughly until all the cellulose has been broken down.  For beans / chick peas this will be 2 – 3 hours on simmer. Lentils will require a little less time, maybe 1  – 1 ½ hours on simmer.  You will know that this has been accomplished when you can see that the legumes have broken open or are soft enough to break open with your fingers.

2.These recipes can be prepared in a crock pot to accommodate busy schedules.  Once you have sautéed the onion, garlic, bacon or pork loin, just toss the ingredient in a crock pot with your beans, chickpeas or lentils and cover with water.  Cook them on low for 24 hours.

3.Any legume may be substituted in these basic recipes to suit your taste.

4.Do not substitute fresh items for pre-packages items.  Example – do not substitute fresh chopped onion for onion power.

5.Play with these recipes, using other fresh ingredients to change the flavor.

  • Example – for a southwestern flavor, add a trace of cumin and 1/8 tsp of crushed oregano to any of the recipes.

6.Ideas on how to serve legumes:

  • Use as a side dish for any chicken or fish
  • Use as a main dish
  • Add more liquid to serve it soup style
  • Serve inside corn tortillas
  • Serve lentils or chick peas over whole or rice pasta

HOW TO LOSE WEIGHT AND STAY THIN

A New Year is upon us.  Fort many of us it is a time to reflect on where we are today and to set new goals for the coming year.  Often those new goals include seeking a healthier lifestyle.

In both the Savoy and Bloomington locations Cosmetic Plastic Surgery Clinic, we receive a lot of questions about how to lose weight and keep it off from patients who are concerned about their increasing weight and those who want to maintain their results after procedures such as liposuction https://www.cosmeticplasticsurgery.com/photo-gallery/results.cfm?Category=5 or abdominoplasty https://www.cosmeticplasticsurgery.com/cosmetic-surgery/tummy-tuck.cfm .  What follows are the basics – that is, bringing a complex subject to a minimum denominator, we can say that what it takes to stay thin and fit is knowledge and a good lifestyle eating ethic.

How many times have you been around someone who tells you they hardly eat anything and that they gain weight, that they can reduce their caloric intake to nothing and that they still gain weight?  While many people would like for that to actually be the truth. The reality is quite different.  The very basic principle is that you will gain weight if you eat more calories than you burn – period, no exceptions.

Have you ever heard people say that if  you exercise enough you will lose weight?  While that is true, the basic principle here is that you absolutely cannot depend on exercise alone to bring your weight down.  It has got to be proportional with a decrease in the amount of food you eat, how that food is consumed and which foods you are consuming.

We will make things as simple as we can to understand what works.

1.     Water intake.  The basic principle of a good lifestyle eating ethic is to consume water.  Here, you must read exactly what it says – water.  Water is not pop, is not tea, is not anything else liquid.  Water is water.  Basic research shows that if you drink a minimum of one glass but better yet two glasses of water prior to your meal, you will consume less food and you will lose weight.  There are many reasons for this to happen, but suffice it to say that basic research has proven this without a doubt.  It is also important to drink water during the day.  In fact, often our bodies confuse hunger with dehydration.  If you have already eaten and are searching for a snack, replace that snack with a glass of water.  Wait 20 minutes.  You may find that your desire for a snack is gone.  Again, I must remark how important it is that it be water – not sweetened water, not flavored water, not anything else other than simple water.  Look around you and you will find that trim, thin, fit people drink only water.

2.     Reduce your portion sizes. Living in a nation where restaurants serve such large portions has been a big injustice to our health and over-all lifestyle.  Over the past few decades, we have begun to consume much larger portions than in the past.  As a result, we are becoming an obese nation with many weight related health problems. The more bad food we consume, the more our bodies crave them. The truth is, the older we get, the less caloric intake our body requires.  Simply put, we must consume less calories in our 40’s than we did in our 30’s, and less in our 50”s than in our 40”s, and less in our 60”s than we consumed in our 50’s and so on.  I suggest beginning to decrease you portion sizes by eating until you are no longer hungry instead of eating until you are full. Decreasing portion sizes can be difficult when you are enjoying your meal, however if you consistently use this rule, you may find it much easier to accomplish.

3.     Avoid salt, sugar, fat and preservatives. Remember that there are enemies in food that you must avoid.  Those are salt, sugar, fat and preservatives.  All of the above are contained in large quantities in fast foods, prepared foods, canned foods (even canned vegetables), packaged foods, etc. While busy schedules may make it difficult for some people to prepare their own meals and go back to a primarily natural source of foods, you must maintain a high alert state for foods that are high in salt, sugar, fat and preservatives.  Remove those enemies from your diet.  One way to avoid these types of foods, even with a busy schedule, is to prepare foods in advance.  Grill meats and fresh veggies in advance, refrigerate or freeze them until needed.  Cook enough brown rice for a few days. Consider adding a fresh veggie, or cilantro, pepper, or a small amount of white raisins and almonds to vary the flavor.  Retrieve it from the refrigerator as needed.  Prepare large quantities of low-sodium soups with fresh veggies, adding fish or chicken etc. if you choose.  Freeze it in portion size containers.  Not only will this method reduce the amount of food enemies in your diet, it will allow you to prepare healthy meals in no time.

4.     Avoid simple carbohydrates. Carbohydrates are not necessarily bad foods; however, simple carbohydrates create a high glycemic state; that is, they increase your blood sugar rapidly which is damaging to many tissues in your body.  It is important to avoid simple carbohydrates and that means sugar and refined grains (reduce to a minimum the amount of bread and pasta and make sure that when you eat these items they are prepared from whole grains).  Glycemic states not only create an easy avenue for your body to store fat, they are also highly damaging to tissues that contain a large number of blood vessels (for example, the retina).

5.     Vegetable proteins.  Increase the amount of vegetable proteins in your diet.  Vegetable proteins offer a great advantage as a food source. They burn slowly, which will keep you from becoming hungry.  You will go for longer periods of time with less hunger.  Vegetable proteins are contained in all legumes (foods that come from a pod) like any kind of bean, lentils, garbanzo (chick peas), etc.  Again you can easily prepare these vegetable proteins in advance and refrigerate or freeze them until needed.

6.     Exercise. Remember to maintain a program of exercise that fits your lifestyle.  That is, don’t create an exercise schedule that requires you to give more time than your schedule allows, it will set you up for failure. It should still allow for the normal routine of your life. That is; work, kids, food preparation, recreation, etc.  The amount of time spent exercising will vary from individual to individual, but it will require anywhere between 30-60 minutes three to five times per week.

7.     Honor the food that you eat. When you are ready to eat, do not do it on the go.  You cannot eat and drive a car.  You cannot eat and watch TV.  You cannot eat and study.  You cannot eat and etc.  That is, when you are ready to eat, drink 1-2 glasses of water then sit down, preferably with family and friends and eat your food slowly.  If you are spending less than five minutes eating your food – you are definitely eating too fast.  When you eat food in small bites it takes you longer to consume your food and you will give time for your body to register that food has been added to your system.  The mechanism of satiation (the feeling of being satisfied) takes some time to develop after food has been added to your stomach.  You also honor and value the food that you eat when you make it an event of importance.  It is one that deserves your full attention.
