Gynecomastia – Large Breasts in Males

Thursday, July 18th, 2019

When males of any age develop large breasts, it can be very damaging to their self-esteem. Though large male breasts are actually quite common, often the result is social isolation. This creates a distressing and debilitating body image that most males try to camouflage with tight upper garments and very loose clothing.

Many of our male patients who suffer from excessive breast tissue, express their inability to really enjoy life. They are embarrassed to take their shirt off.  It constricts their willingness to be social and participate in sports. They are unable to enjoy vacations at the beach and most summer related activities.  Most importantly, it changes their ability to interact within key relationships in their lives.  Many males find it difficult to feel masculine during intimate moments with their partner. They find that it even inhibits the way they interact or play with their children.

Male breasts are the result of increased volume of both fat and glandular breast tissue.  At Cosmetic Plastic Surgery Center, our patients learn that male breast tissue is indeed an occurrence in 100% of males. However, this tissue is vestigial, that is, it is present in very small amounts in most males  For reasons associated with hormones, genetics, diet, medications and environmental factors, the amount of both fat and breast tissue can increase in the breast area of males of all ages (aging men, young men, children alike…).

The treatment for this problem is both effective and simple.  Very often patients ask me, “Dr. Castillo, can this problem be solved with liposuction only?”  The answer is a sometimes. Sometimes the case is such that liposuction is all that is needed to reduce the size of the breast and deliver a great result.

The treatment is more often two-fold.  It involves the removal of fatty tissue from the pectoral area through liposuction with very small cannulas. We do this using traditional tumescent liposuction techniques and ultrasonic-assisted liposuction. Then we create a small, invisible incision in the nipple- areolar region. This incision is utilized as a window. With special, delicate instruments, we use this window to remove a significant portion of breast tissue which in most individuals is not accessible for removal through liposuction, because it is very thick, fibrous, glandular tissue.  It is removed by dissection with special instruments that create no injury to the surrounding tissues.  The entire procedure takes two hours, and cleanly and effectively removes all of the enlarged breast and fatty tissue from the area.

A fitted garment is worn underneath normal clothing for the first week after surgery. Very small sutures are removed within six days after the procedure. The patient is allowed to do routine light physical activities within three days after the procedure.  This procedure, known as excision of male Gynecomastia will help to resolve the lack of self-confidence that can occur after years of teasing and scrutiny. It will reveal a more masculine male contour of the chest. It has been universally successful and rewarding procedure. The changes in self-esteem and positive personality reinforcement are seen immediately.  It has been a most satisfying way of correcting a rather disturbing problem for our patients.

If you would like further information on this subject, call our office to schedule a consultation. During consultation, we can discuss your desires and concerns. You will be able to view before and after photos of other patients who have undergone this procedure.  We will answers any questions you may have and give you the information needed to help you decide if this procedure is right for you.

POST-SURGICAL PAIN PUMPS

Friday, October 26th, 2018

A common reasons patients put off having the cosmetic surgery they have always dreams of is their concern about post-surgical pain. While pain is a valid concern, it doesn’t have to be. Today, we offer our patients a simple way to manage pain without pills (narcotics).

This technology is called a pain pump. While the majority of the surgeries we perform at Cosmetic Plastic Surgery do not results in much post-surgical pain, utilization of a pain pump can offer a more comfortable recovery period for those procedures that do or for those individuals who have a low pain threshold.

A pain pump is a simple, non-electrical device that delivers local anesthetic (numbing

medication) such as Marcaine, directly to the surgical site after your surgery, allowing you to recover from surgery without the typically discomfort.

The pain pump looks like a small ball or balloon and comes in it’s own strapped carrying case. Dr. Castillo connects the pain pump at the surgical site at the time of surgery. It will deliver medication to the surgical area slowly and continuously through tiny catheters for 3-4 days. As the patient, you do not have to do anything with the pain pump. It is automatically regulated; you just get to reap the rewards of steady relief!

The pain pump offers pain management without the side effects of traditional methods of pain control like oral pain medications. This means you can avoid the potential side effect that oral pain medications often cause such as:

      •mental fogginess             •drowsiness              •nausea             •vomiting             •dizziness            •constipation

For patients who are narcotic sensitive or allergic, this can be a first-rate alternative for pain management. Patients who elect to use pain pumps are typically moving, driving and functioning and return to normal life activities more quickly since the affects are limited to the surgical area, the rest of your body & mind remain unaffected and have normal function.

While it does add about $350.00 to the cost of your surgery, the feedback we receive from our patients is that use of the pain pump provided them with a much smoother and quicker recovery than they had originally anticipated.

Ask Our Expert: Ways To Actually Gain Confidence With Male Gynecomastia

Wednesday, July 18th, 2018

Does Dr. Castillo offer a procedure for males with large breasts?

Dr. Castillo:  Correcting large male breasts is one of the most life changing cosmetic surgeries for men. For some men, having large breasts significantly affects the way they see themselves, resulting in a depletion of self-esteem. It changes the way they relates physically and emotional to others.  It changes how they play with their children and respond to affection from their wife or partner.  It reduces their level of activity and willingness to be social.

BEFORE Male Breast Reduction

Does this happen to a lot of men?

Dr. Castillo: Yes, much more often than one would think. The over-development of male breast tissue, known as Gynecomastia is  a relatively common condition and can occur at any age.  It can be related to genetics, hormonal changes, rapid weight gain or the use of certain drugs. It is easy to correct with breast liposuction or the surgical removal of undesired breast tissue through a small incision or a combination of both methods.

AFTER Male Breast Reduction Surgery by Dr. G.D. Castillo

When can they return to work?

Dr. Castillo:  Light work duties can typically be resumed with in three to four days after male breast reduction surgery, while heavier activities can usually be resumed after the first week.

What are the age limits for male breast reduction?

We perform breast reduction by liposuction as early as age 14.  If a young male (age 14 and up) has had an endocrinology study completed that indicates the need for surgical tissue removal then in those instances surgical breast reduction would be performed.

What makes you qualified to perform these procedures:

Dr. Castillo:  I am Triple-Board Certified and have been performing cosmetic surgery including male breast reduction for more than 3o years. I am the Past-President of The American Society of Liposuction Surgery and The Past-President of The American Academy of Cosmetic Surgery.

Breast Asymmetry

Friday, June 10th, 2011

Not All Breasts Are Created Equal

Breast asymmetry is quite common, though the degree and patient perception of it varies.  Often patients are not aware of their asymmetry, they just recognize that they are unhappy with the appearance of their breasts.  Frequently women have one breast that is larger than the other breast or nipple that is positioned lower than the other.  Patient’s often share their stories of frustrations when searching for clothing that will fit properly without accentuating the asymmetry and the continual distress it causes them.

If breast asymmetry is so common, why does it bother us females so much if our breasts are not perfectly symmetrical?  Maybe as women, we just haven’t had as many opportunities to compare our breasts to the breasts of the average women unlike say… women living in other countries, like maybe…France, where breasts seem to be on display. Maybe it’s because we torture ourselves by thinking our partner is comparing our breasts imperfections to the perfectly perky, perfectly shaped, perfectly sized, perfectly air brushed breasts in magazines.  Maybe it is because we remember what our breasts looked like before children. Maybe it’s because we just want to be the best self we can be. The list could go on indefinitely I’m sure. Unfortunately, there isn’t much comfort to be found in the idea that there are lots of women facing the same imperfections.

Fortunately, Dr. Castillo has decades of experience in dealing with and developing solutions for this very problem.  For those of you who have not had the pleasure on meeting G.D. Castillo, M.D., “as a surgeon (and an individual) he is a breath of fresh air” as quoted by one of our patients.  I must agree!  He is a truly gifted surgeon with a compassionate, understanding, engaging bedside manner. Dr. Castillo’s goal is your goal: to create beautiful breasts that look and feel exactly as you envisioned them.

At Cosmetic Plastic Surgery Clinic, we offer options to address each type of asymmetry concern:

1.     Breast augmentation – This procedure may involve inserting a breast implant in one or both breasts, depending on the individual patient’s needs.

2.     Liposuction – Many times liposuction of the larger breast can be used to simply equal out the volume / size of your breasts without breast implants.

3.     Breast Reduction – This cosmetic surgery procedure can be used to reduce the size of one or both breasts to create breast symmetry.

4.     Breast Lift (Mastopexy) – This breast surgery is used improve breast ptosis (sagging) in one or both breasts, reshaping the breasts to create beautiful symmetrical breasts.

5.     Nipple Reduction or Nipple Augmentation – This cosmetic procedure can be used to increase, decrease or equalize the size and shape of the nipple and give balance to the breasts.

6.     A combination of one or more of the above breast surgeries is sometimes needed to correct the asymmetry in its entirety.
