Buyer Beware: Billboard of Half Truths

How many of you have seen the billboards or other material that states “Vitamin D from Tanning Beds and Sunshine can reduce your risk of Breast Cancer by 75%”? It even displays a pink ribbon that mimics the pink ribbon used by the American Breast Cancer Association. Look closely…DO NOT BE FOOLED.  Though similar, it is not the same pink ribbon of support used by the American Breast Cancer Association, nor was the ABCA in support or even aware of this marketing campaign when I called them. The American Breast Cancer Association doesn’t make a recommendation to use a tanning bed or sun bathe. Touting tanning as a measure to prevent breast cancer is not only scientifically unfounded by peer review, but is incredibly self-defeating to those who heed the tanning industries misleading advise.

Those involved in this marketing campaign, extracted theories from on-going studies that report that there is some link between Vitamin D Deficiency and certain cancers such as breast cancer, along with other disorders including but not limited to depression, autism, and osteoporosis and so on.  Please understand these studies have not concluded that Vitamin D Deficiency is a cause of illnesses such as breast cancer.  They are only stating that there is evidence that those with certain illnesses are often also Vitamin D deficient. They then crossed that information with the fact that one way to increase vitamin D is through exposure to sunlight or a tanning bed and came up with an unconscionable advertisement.

Vitamin D levels can be increased through various options.  Some are safe.  Some are not.  Some options have a positive effect on the body in general while other options are scientifically proven carcinogens.  I don’t know about you, but it doesn’t make much sense to me to expect to lessen my chance of developing breast cancer by using a method (tanning) that is proven to cause other forms of cancer such as deadly skin cancers, cancers of the eye, other health issues and pre-mature aging (wrinkles, brown spots, broken blood vessels, sagging, discolored skin).  How about you?

The safe options to increase Vitamin D levels include:

  1. Oral calcium supplements that include vitamin D.
  2. Diet – according to the Office of Dietary Supplements (NIH) including salmon, tuna, beef liver, eggs, orange juice, milk, yogurt, and some cereals in your diet will increase your vitamin D intake naturally.

Options to increase vitamin D that are proven to cause cancer and pre-mature aging:

  1. UV radiation from the sun
  2. UV radiation from tanning beds

Earlier this summer, Lancet Oncology announced that the rating on tanning beds has been moved from a “probable” cause of skin cancer to a “DEFINATE” cause of skin cancer.  This means that tanning beds are now a PROVEN carcinogen, just like cigarettes / nicotine.  UV radiation not only makes you look “old” before you time, it can KILL you.

According to statistics provided by Dr. Brooke Rutleadge Seckel, Assistant Professor of Surgery Harvard Medical School, in 1935, Americans had a 1 in 1500 risk of developing invasive melanoma in their lifetime.  By 2001 that number increased to 1 in 74.  Today, it is 1 in 50!  Over ONE MILLION new cases of squamous and basal cell skin cancers will be diagnosed this year along with more than 60,000 cases of Melanoma (the deadliest of skin cancers).

I understand that many people are convinced that in order for their skin to look attractive, it must be tan, have “some color.”  But, I think you’ll have to agree that no matter how tan your skin is, there is nothing SEXY about a cancerous lesion on your face! Healthy, radiant skin of any color is beautiful skin.  This means skin that is soft, supple, well hydrated and free of discolorations such as brown spots, yellowing, and broken vessels. One can have a nice bone structure, a bright smile, a bubbly personality yet nothing is as salient as glowing, undamaged skin.

I am no way judging tanning salon owners for their choice of business or those whom choose to tan.  Every adult has the right to make that choice.  I am however, objecting to the attempt to mislead the public for an industries own growth.  Every adult has the right to freedom of choice.  I just think you also have the duty to be correctly informed of the benefits and consequences of any choice.  I believe it to be a sad day in America when those affiliated with this campaign and industry (tanning) are willing to show such little regard for human health…for human life…

Gynecomastia – Large Breasts in Males

When males of any age develop large breasts, it can be very damaging to their self-esteem. Though large male breasts are actually quite common, often the result is social isolation. This creates a distressing and debilitating body image that most males try to camouflage with tight upper garments and very loose clothing.

Many of our male patients who suffer from excessive breast tissue, express their inability to really enjoy life. They are embarrassed to take their shirt off.  It constricts their willingness to be social and participate in sports. They are unable to enjoy vacations at the beach and most summer related activities.  Most importantly, it changes their ability to interact within key relationships in their lives.  Many males find it difficult to feel masculine during intimate moments with their partner. They find that it even inhibits the way they interact or play with their children.

Male breasts are the result of increased volume of both fat and glandular breast tissue.  At Cosmetic Plastic Surgery, our patients learn that male breast tissue is indeed an occurrence in 100% of males. However, this tissue is vestigial, that is, it is present in very small amounts in most males  For reasons associated with genetics, diet, and environmental factors, the amount of both fat and breast tissue can increase in the breast area of males of all ages (aging men, young men, children alike…).

At Cosmetic Plastic Surgery, the treatment for this problem is both effective and simple.  Very often patients ask me, “Dr. Castillo, can this problem be solved with liposuction?”  The answer is a definitive no. The treatment is two-fold.  It involves the removal of fatty tissue from the pectoral area through liposuction with very small cannulas. We do this using traditional tumescent liposuction techniques and ultrasonic-assisted liposuction.

Then we create a small, invisible incision in the nipple- areolar region. This incision is utilized as a window. With special, delicate instruments, we use this window to remove a significant portion of breast tissue which in most individuals is not accessible for removal through liposuction, because it is very thick, fibrous, glandular tissue.  It is removed by dissection with special instruments that create no injury to the surrounding tissues.  The entire procedure takes two hours, and cleanly and effectively removes all of the offending breast and fatty tissue from the area.

A fitted garment is used and worn underneath normal clothing for the first week after surgery. Very small sutures are removed within six days after the procedure.The patient is allowed to do routine light physical activities within three days after the procedure.  This procedure, known as excision of male Gynecomastia will help to resolve the lack of self-confidence that can occur after years of teasing and scrutiny. It will reveal a more masculine male contour of the chest. It has been universally successful and there is a remarkable lack of problems associated with it. The changes in self-esteem and positive personality reinforcement are seen immediately.  It has been a most satisfying way of correcting a rather disturbing problem for our patients.

Q:  I am considering breast augmentation. How do I know which size implant I should chose?

A:  Making the decision about breast size is a very personal decision. If you have not already had a consultation with a reputable cosmetic plastic surgeon, I recommend that you talk with one. Your surgeon and his staff should be able to answer many of your questions about implants and sizes. They should be able to talk with you more about how to select the right sizing with your particular body shape while keeping your goals in mind. Here are 3 options that can help you make a visual decision about sizing.
OPTION #1:
The majority of surgeons use “the baggie of rice method,” although Dr. Castillo offers a much more realistic option for his patients in both the Savoy (Champaign – Urbana) and Bloomington offices. To utilize the at home baggie of rice method, you can put rice, oatmeal, or birdseed in a sandwich baggie. Do not use a zip lock baggie as they are too firm. (1 ? cups = 10 ounces or 1 ? cups = 12 ounces…) Most breast augmentations utilize between 9 – 14 ounces for each breast (270cc – 420cc per breast). You should probably start with 10 ounces then adjust as you prefer. Select a bra for the size you want to be. Put the filled baggies inside the unpadded bra. Once you find the size baggie you are comfortable with, you may want to wear it for a few hours, with different shirts, do some normal daily activities…this will help you decide if the size is truly comfortable.
OPTION #2:
In both our Savoy and Bloomington offices, Dr. Castillo provides the option of trying on professional sizer implants in the office. This option by far offers the most natural, realistic visual comparison.  We have a variety of sizes so that our patients can get a good idea of what their results will look like.
At the time of the consultation, we give the patient an unpadded bra. The patient can also bring one if she knows about what size she would like to be. I suggest that our patients bring 2 different style tops; one form-fitting, one loose fitting. This gives our patients a great feel for how each size can change their body appearance. I have viewed the DVD. I think it would be helpful for anyone who is considering breast implants. The DVD introduces you to a patient who has Natrelle breast implants as she shares her experience with her daughter who is now considering breast enhancement. You will view some before and after photos of breast augmentation. More importantly, the DVD takes you through a consultation so you know what to expect. I think this kit will be helpful in preparing you to ask the right questions when you are talking with your surgeon.

I would recommend using your own unpadded bra with the sample implants. The sample implants from this kit allow you to try them on with different pieces of clothing in the privacy of your own home. This will help you get a good idea of what size implants you will be comfortable with. In my opinion, they do not give near the realistic visual as the professional implants that we have in the office. The implant samples from the kit do not provide as smooth or realistic shape as our profession implant provide.  Therefore, even if you purchase the kit, I would still recommend taking advantage of the profession sizer fitting at our office.

Mommy Make-Over

As little girls, we’ve all watched enough Disney to know that, just in the nick of time, our fairy god mother arrives, waves her glamour wand and poof – happily ever after begins.
Then we have our first child and reality sets in. Who knew such beautiful little angels would do such unspeakable things to our bodies! If you have ever thought “Where is my Fairy Godmother now?” you’re not alone. I think most of us have stood in front of a mirror months or even years after giving birth in hopes that Fairy Godmother will arrive and grant our wish for a mommy make-over! Just like Cinderelli, we take care of everyone around us. We “make the fire, fix the breakfast, wash the dishes, and do the mopping…”
As moms we give so much of our time, energy and love to everyone around us that we often forget to take care of ourselves. We’ve all heard the term “kids can be cruel.”  We just didn’t know it was in reference to what was about to happen to our bodies after pregnancy! We are left with sagging skin where our waist used to be. Breasts are now distorted and stretch out. There is weight gain of the hips and thighs. Our new saddlebags become a quick reminder that we are probably no longer eligible for low rise jeans.
For most of us, pregnancy takes a huge toll on our body. With child-bearing, raising a family, car pool, homework, household chores, not to mention sun damage, age and heredity hovering over us, our own well being and self development can get lost in the shuffle. We are so busy that we don’t even notice until someday, a friend or even a complete stranger makes a comment that really sticks.
Perception is reality. How we view ourselves and how we think others view us determines much of how interactive we are with those around us.  All of the changes we see on the outside can have some damaging effects on the inside as well. We lose self-respect, confidence and spirit.  We become less socially active. Sound familiar?
Since you are reading this blog, I am going to assume there is something about your body that makes you uncomfortable. What is it that bothers you? Do you find yourself repeatedly thinking about a particular area of your face or body?  How many times have you thought about it today? In the last 6 hours?
Maybe it’s your abdomen. Maybe it just hasn’t recovered after the birth of your second child. Often, it doesn’t matter how health conscious and physically fit you are. You cannot exercise away the stretched out, saggy skin hanging from your abdomen that once provided a safe place for your precious bundles of joy to develop. Maybe you find yourself so self-conscious that it is becoming more and more difficult for you to expose your body to your partner. You’ve traded in your bikini for a one piece with a skirt. You find yourself selecting styles of clothing that attempt to hide your body instead of ones that say you are vibrant, assured women.
Maybe you are a single mom who has a strong desire to begin the search for your prince charming. But, saying yes to that first date has you terrified. You don’t feel good about the changes that have occurred in your body. That stops you from coming across as the fun, outgoing, intellectual that you are. Maybe it’s the uncontrollable, extended greeting your arm gives; you know, when the underside of your arm continues to wave to your friend long after you’ve stopped. Maybe you’re an empty nester now and you would like to finally do something for yourself.
Many women are no longer waiting for their fairy godmother to wave her magic wand. They are taking charge, no longer accepting that their distorted bodies are the price they have to pay for having a family. I guess you could say that they are asking Dr. Castillo to wave his magic scalpel instead.
Often, Mothers walk into our office for the first time with their shoulders hanging low. They are reserved, tired, insecure. By the day of their surgery, a change has already begun. It takes courage to take charge and force a change. Of course they are a little nervous. But they are so excited! How many people do you know who head off to surgery with a big smile on their face?  After their “mommy make-over,” they become renewed. They are suddenly reunited with the person they used to be.  Their families have miraculously survived during the recovery period. They have a new brand new attitude. They are energetic. They laugh more. They smile more. They walk straight up, looking out at what the world has to offer.
Cosmetic surgery is not the right choice for everyone. If you are considering it, do your homework. Ask lots of questions at your consultation. If you do not feel comfortable during the consultation, seek out other doctor. You need to develop realistic goals. Having cosmetic surgery will not fix your problems. However, substantial changes can occur in patients who choose cosmetic surgery for the right reasons. It often changes their quality of life. In many cases, patients become motivated to live a more health conscious, socially active lifestyle.
As mothers it’s important to remember that we are also individuals. It’s okay to be a mom and still want to be sexy, current and confident. In fact, who could possibly be a better role model for your children than a self-assured, strong, healthy, enthusiastic mommy?
