Ask Our Expert: CO2 Micro-Fractional Laser Resurfacing Of The Upper Lip…

Question: With the CO2 Micro-Fractional Procedure above the lip, would there be any anesthetizing, of the area for pain? Also, does it peel or just turn red?  And Lastly, what days of the week does Roxanne do this procedure?

Answer:
Karen, when we perform more aggressive CO2 Micro-Fractional Laser Resurfacing to on lines around the mouth, we utilize three forms of discomfort control:
    1.  We administer a Tylenol with Codeine upon your arrival to the appointment so you will need a driver.
    2.  We apply a topical numbing ointment.
    3.  Once the above two forms have taken effect (about 30-40 minutes) Dr. Castillo will do a regional nerve block. This is of a few injections similar to having your mouth numbed at the dentist. 
REDNESS AND SWELLING: You can expect redness and swelling of the upper lip area.  It will be worse the day after treatment.  Typically by day 3, the swelling will start to resolve.   Since we would only be treating the upper lip instead of the whole face, your swelling is likely to be quite minimal.  You can expect to be red then pink then you’ll return to your normal skin tone.  A light pink color may last a few weeks depending on the level of aggressiveness we use.
PEELING: You will begin to peel within 3-4 days and the peeling typically lasts 48 hours. You will be able to cover with make-up within 4-6 days after treatment. 
I typically perform this procedure Tuesday – Friday.  Please let me know if you have further questions.

Wrinkled Or Not, Here I Come

Wrinkled was never  what I dreamed of becoming when I grew up; a zoo keeper, a talk show host or entrepreneur,  sure, but I checked carefully and wrinkled has NEVER appeared on my list, not even once!

We had some new staff photos taken recently at Cosmetic Plastic Surgery Center.  When we got them back I was startled to see a slightly older version of me in the photos.  I was forced to acknowledge that I too am slowly creeping up on a time in life when minor changes in my facial structure are beginning to occur.  When I look closely in the mirror, I can now see a hint of my mom looking back at me.  I know there will come a time when decisions will have to be made.  Will I decide to age gracefully or choose to defy it with a facelift.  Let’s see.  I better check my list one more time just to be sure.  Nope, still the word “wrinkled” does not appear.  Defy it is then.

How does one do that you ask?  Well it is easier if you have stayed out of sun, but for most of us that is not the case.  So that leaves protect and repair.  Both my parents were diagnoses with skin cancer while I was in my mid to late twenties, so really just a few years ago. ; -)  Hey… there is no judgment here ok.  This is my blog. Besides, like I always say “if age is just a number than I would prefer to keep mine unlisted.”

Watching the anguish my mother endured when the tip of her nose had to be removed, leaving it disfigured and later when my dad who had a large cancerous lesion in the center of his forehead, I realized the true dangers of the sun. Protection is a must when abstinence isn’t possible.  (from the sun!)

At that point, I began a daily regimen of sunscreen and swore off tanning.  That alone has bought me some time.  I am faithful to my medical skincare regimen.  I don’t jump around trying the latest and greatest infomercial product.  That’s a tough one for most of us – advertising can be so convincing.  I stick with medical grade skincare products only, never anything over-the-counter.  That’s not to say that there aren’t good OTC products out there, it is just that I value my time and money more than that and refuse to spend time and money hunting for something that “might” do the trick when I can go straight to medical grade skincare and feel confident that my investment will pay off.

I was recently telling a girlfriend that I am considering having full face laser resurfacing in the fall.  She responded “For what?  You don’t have a wrinkle on your face.”  Which of course isn’t quite accurate and to which I responded “yes, and I’d like to keep it that way.”  I explained to her that I am a firm believer in preventative (early) maintenance for multiple reasons.  First, addressing aging earlier rather than later gives me the opportunity to enjoy my results for as long as possible.  Why wait until “I am bad enough or old enough, or wrinkly enough to justify making improvements.” The only thing that will accomplish is that my self-esteem will take a hit.  I have always believed in taking care of myself with exercise, healthy food choices, and a clean lifestyle.  I want to enjoy the rewards of my efforts.  What if you just found out your home had a few termites, would you wait until the foundation was ready to collapse before something about it? Of course not, so why should our bodies be any different?

I believe early maintenance is beneficial because I think it provides more privacy.  The more there is to repair, the more dramatic the change can be.  Friends and family are less likely to suspect cosmetic surgery and more likely to assume that I am more well rested or that my new hair cut just took 10 years off my appearance.   Well… unless you write a cosmetic surgery and skin rejuvenation blog and tell the whole world about your anti-aging game plan… that does tend to cause some suspicion.

 

So will I have a facelift?  Well if the little things I do along the way; medical grade skincare, exfoliation treatments, Botox Cosmetic, laser resurfacing aren’t enough to keep the peace within myself, then, yes, I would consider a facelift when the time is right.  I would choose a traditional facelift method.  I am not interested in entertaining the idea of something like the Lifestyle Lift.  I have witnessed too many patients who have come to our office because they had the Lifestyle Lift somewhere else and it didn’t turn out as they hoped.  Then they need additional surgery (a proper facelift)  to correct it, something we never see with traditional facelift surgeries by Dr. Castillo.

Have you have had that shocking moment where you stumble out of bed, make your way toward the shower, pass the mirror, catch a peek on the way and see…what…rub your eyes to get a more clear view, fumble for your spectacles, say a quick prayer, and head back for another look in the mirror… “what?  How can that be??? Chest wrinkles?  Chest wrinkles!  Really!  Oh come on, you’ve got to be kidding me.  This is not the best way to start a day.”

Score: Mirror – 1, Self-Esteem – 0.

Even those of us who have the greatest capability for denial somewhere down deep recognize that someday, far, far away of course, but some day, we will develop a few lines or wrinkles on our face…but on our chests??  Is this some kind of cruel joke?  “Is this nature’s way of punishing me for that time I didn’t return the shopping cart to the cart coral at Target because it was raining and I didn’t want my hair to turn into a frizz ball?”   Rest assured (though I know this is no real comfort) at some point most of us have a moment, maybe not a chest moment, but a moment where some part of the aging process knocks us off our feet for a bit.

The good news is science is prepared to re-inflate the bubble that nature just popped.  Bless the brilliant men and women who have dedicated their lives to creating and improving laser technology. There should be a holiday, maybe even two, named after them don’t you think?

The other good news is you are not sentenced to a life of hiding your wrinkled cleavage.  There are a few ways to correct the damaged skin of your chest.

1. Most importantly, PROTECT your chest from UV radiation.  Use a medical-grade, broad-spectrum, physical sunscreen that contains Zinc Oxide, Titanium Dioxide or both every day of the year. I recommend a sunscreen called Sun Defense by Castillo MD Skin Science.  It offers effective sun protection consisting of a 12% Zinc Oxide and 7.5% Octinoxate for both our cosmetic surgery and skin rejuvenation patients.

2.  Stay away from tanning beds, period.  They are the enemy!  UVA rays kill elastic fibers and will only create more damage to your skin.  Tan skin IS damaged skin.  Damaged skin will result in wrinkles eventually.

3. Use UV protective clothing and a wide brimmed hat to provide additional protection.

4. Talk with a board-certified cosmetic surgeon who has extensive experience with CO2 Micro-Fractional Laser Resurfacing on the chest.  (Just a side note: Dr. Castillo was one of the first Cosmetic Surgeons in the Mid-West to offer this procedure.)  Micro-Fractional Laser Resurfacing will decrease the lines of your chest (décolletage) while building collagen and tightening the treated skin.  It will also make a significant improvement to the texture of your skin and decrease or remove your brown spots and discoloration. In some cases more than one treatment may be necessary due to the delicate state of the chest skin.

5. Medical Grade skincare products – There are effective medical grades skincare products created to specifically treat the chest area.  These products will not solve your problem alone but work well to support and prolong the effects of CO2 Micro-Fractional Laser Resurfacing on the chest.

Say Goodbye to Chest Lines, Wrinkles and Brown Spots!

I was speaking with one of Dr. Castillo’s breast augmentation patients during a skin rejuvenation consultation.  She explained to me that a few years back she had breast enhancement https://www.cosmeticplasticsurgery.com/cosmetic-surgery/breast-enlargement.cfm in Dr. Castillo’s Cosmetic Surgery facility in Savoy, Illinois and that she couldn’t be happier with her results and the entire Cosmetic Plastic Surgery experience.

However, she has some fine lines and wrinkles on her chest between her breasts along with a moderate amount of brown spots. She expressed that she would like to show off her new cleavage https://www.cosmeticplasticsurgery.com/photo-gallery/results.cfm?Category=10 without being embarrassed about having wrinkles between her new, perky breasts.  She asked if G.D. Castillo M.D. and Cosmetic Plastic Surgery Skin Restoration and Laser Institute offered skin treatments to remove the wrinkles and brown spots of the chest.

I explained to her that the vertical lines and brown spots she was describing are common among those who have been over-exposed to the sun. The sun breaks down elastin and collagen of the chest, creating lines, wrinkles, and sagging skin.  It also causes melanocytes (pigmented cells that create brown spots she is seeing on her chest) to over-populate the surface of the skin in attempt to protect itself from damaging UV radiation.

I explained to her that yes, we can help with her chest lines and brown spots.  To improve the look of her damaged skin, first, she would need to begin to protect her décolletage using medical grade broad-spectrum sunscreen 365 days per year.  UV radiation will create more severe damage to areas such as the chest, neck and hands. The characteristics of the skin of the chest, neck and hands simply cannot tolerate the same amount of UV radiation as the face. The skin of the chest is not as forgiving as skin surfaces such as the face when it is repeatedly over-exposed to the sun. The skin of the chest is thinner, containing less elastin / collagen.  As the elastin is broken down, lines, wrinkles and sagging of the skin WILL occur.

Unlike the face, skin of décolletage does not contain an abundance of oil glands.  Why is this a problem?  Having an abundance of oil glands is important because as oil glands cells divide, they can become almost any other type of epidermal cell.  Therefore, when cells get damaged, they provide new cells to help to restore your damaged skin.

The good news is there are a few ways to correct or at least improve the skin of your chest.

  1.  First, and most importantly, PROTECT your chest for the sun.  Use a medical-grade, broad-spectrum, physical sunscreen  every day of the year. I recommend one of two  physical sunscreens:  Sun Defense or Prime Defense By Castillo MD Skin Science.  Both have high percentages of physical block ingredients (Titanium Dioxide and / or Zinc Oxide) making them superior  choices for sun protection.
  2. Avoid tanning beds as UVA rays will only create more damage to your skin.
  3. Use UV protective clothing and a wide brimmed hat to provide additional protection.
  4. Consult G.D. Castillo M.D. or another Cosmetic Plastic Surgeon regarding the following treatment options:
    1. CO2 Micro-Fractional Laser Resurfacing – This will decrease the lines of your chest (décolletage) while building collagen within the treated skin.  It will also make a significant improvement to the texture of your skin and decrease or remove your brown spots or discoloration. More than one treatment may be necessary due to the delicate state of the chest skin.  Did you notice that I highlighted, underlined and italicized the “CO2” in CO2 Micro-Fractional Laser Resurfacing?  That is because CO2 is the key there!! Medical Microdermabrasion – Multiple microdermabrasion treatments https://www.cosmeticplasticsurgery.com/cosmetic-surgery/skin-resurfacing.cfm will soften the appearance of your chest lines and lighten the brown spots over time.  Be aware that this treatment will not build collagen, provide tightening or remove the lines and wrinkles on your chest.  It is meant only to brighten the skin by removing the build-up of dead skin cells.
    3. Medical Grade skincare products – There are effective medical grades skincare products created to specifically treat the chest area.  These products will not solve your problem alone but work well to support and prolong the effects of the above mentioned treatments. Medical grade means that they are of a higher strength and must be over seen by a physician therefor these are not your average over-the-counter disappointments.

Ask Our Expert: Retin-A

RetinA postQ:   My dermatologists gave me a prescription for Retin-A, but after a few days my skin became very red and irritated.   My Doctor explained that several weeks of skin irritation is often just par for the course when using Retin-A.  He asked me to try applying it every other night instead of every night, to be patient and that eventually the irritation would resolve itself.  I stopped using the Retin-A because the irritation was worse than my aging skin.   Is there anything else I can do?

A:  I am happy to report – YES!  Both our Bloomington area and Champaign area Cosmetic Plastic Surgery Clinic patients have found success with a few simple adjustments to the way they apply their Retin-A, Renova, Tretinion, etc.

1.  After you cleanse your face, pat it dry and wait at least 15 minutes (to assure your skin is completely dry) before applying your Retin-A.

2. Apply a pea size amount.  This is typically all you need for the entire face. Remember that more is not better in this case. 

3. Avoid combining other exfoliating or skin regenerating products like glycolic acids, with Retin-A, at least until your skin is able to easily tolerate the Retin-A.

4. Always apply a moisturizer on top of your Retin-A.  This is a very important step in the success of your regimen so resist skipping it. (If you find that you are still experiencing dry, irritated skin after trying all of the suggestions here,  you can try mixing your moisturizer and Retin-A together as this will reduce its potency. )

5. Begin applying your Retin-A every third night.  As your skin tolerates the Retin-A, increase use to every other night, then every night.

6. Remember that Retin-A and like products will cause increased sensitivity to the sun.  Therefore Retin-A should only be applied at night only, never during the day.  Wear a medical grade broad-spectrum suncreen every day, re-apply the sunscreen when needed and avoid excessive sun exposure.

If despite all of the above suggestions, you find that you are still unable to tolerate Retin-A, you may want to consider medical microdermabrasion.  It is a pain-free, even relaxing way to keep your skin well exfoliated and looking fresh.  For more information on medical microdermabrasion.

Barefoot Beauty

There are two things a girl should never miss; a great shoe sale, and her nightly skincare regimen.  My most prized possession on earth, without a doubt, is a serious collection of must have heels. There is just something about shoes…oooh…they are just so good for my soul! (I didn’t say “sole,” as that’s a whole other story) I find that even on the worst of days, I can walk in to my shoe closet and find something to put a little sass in my step. As Marilyn Monroe so eloquently put it… “give a girl the right pair of shoes and she can conquer the world.”  So true!

However, if I ever had to choose between giving up my skincare regimen and giving up my beloved shoe “investment,” I would be going barefoot; sad, but barefooted;  And…well…shorter.     ….But I’d have beautiful skin!!

Now I know we’re all inundated with cosmetic commercials telling us that all we have to do is apply their make-up and “wha-la,” you will have flawless, radiant, model-esk skin and everyone will sit up to  take notice as you enter the room. But the reality is flawless radiance doesn’t begin with cosmetics.  It begins with skin and how we take care of it. Proper care and maintenance is the only path to achieve that. The best make-up in the world won’t give you the flawless, smooth complexion you’re looking for if you’re battling dull, dry, flaky, sun-abused skin. A consistent, daily skincare regimen will improve the texture of your skin, getting you yet another step closer to your goal.

So start with the basics … And remember… success can ONLY be accomplished if the basics are part of your DAILY routine…

  1. Cleanse – Clean your skin with a cleanser for your skin type.  The right cleanser should not leave your skin feeling dry and tight. It should feel fresh and clean.  I know this next part is tempting after al one day but – do not go to bed with your make-up on. Pinky swear?!
  2. Exfoliate – Proper exfoliation is a must.  If you develop a dead skin build up, your skin will look dull and develop a yellow tinge. Lines, wrinkles and poor texture will become more noticeable and your make-up will not lay properly on your skin.  You can accomplish this task in several ways.  I recommend monthly medical microdermabrasion treatments to remove dead cells and brighten your skin along with a nightly application of Retexturing Complex  or Retinol Serum PM by Castillo MD Skin Science.  I also love, love, love  to use a weekly application of the Dr. Castillo’s Microdermabrasion Scrub.  It leaves my skin so smooth.
  3. Hydration – Choose a moisturizer for your skin type.  Proper hydration is imperative when it comes to creating smooth, fresh, supple skin.
  4. Protection – The first three steps on your path to sensational skin will mean nothing without providing UVA/UVB sun protection, 365 days per year.

I referred to steps 1-4 as the basics.  That is because in some cases, you may need to add a treatment to your skincare regimen as well.  This may be in the form of particular active ingredients to enhance your basic routine such l. It could also include of series of IPL (Intense Pulse Light), Medical Microdermabrasion or even CO2 Micro-Fractional Laser Resurfacing, just depending on the severity of your skin concerns and your goals.

Beautiful Skin…It’s NEVER Too Late

Beautiful Skin

Awhile back, I was exiting the movie theater in Savoy, Illinois when I heard a woman’s voice.  “Excuse me, Miss.  Miss, excuse me.”  I turned around as a lady put her hand on my forearm and said “I’m sorry to bother you but I just had to tell you that you have beautiful skin.”  Clearly an extremely intelligent woman, she then turned to the gentleman whose arm I was on and said “and you sir, are very lucky!” I couldn’t have paid for a moment as fabulous as that one (lol)!  I still smile and giggle a little every time I think about it.

In that instant I knew that all the nights I had stayed up an extra five minutes taking care of my skin instead of just hopping into bed had paid off.   Just last week I was pushing my shopping cart through Meijer’s when a gentleman stopped, looked at me and said very sincerely, “you have the most beautiful skin.”  I blushed, thanked him for his compliment and went back to the mundane task of grocery shopping, only now with a lifted spirit and smile on my face.

So am I one of the “lucky” ones, just born with great skin?  I don’t think so.  I had thin, dry skin most of my life.  In fact, as far as luck goes, I don’t believe there is such a thing.  I have always believed that “luck” is the bi-product of being prepared.  It is about applying effort to those things that are important.  Having “beautiful” or healthy skin is no different.  I am telling you this story because I want you know that if you are bothered by the condition of your skin, it’s not too late to make an improvement that can really make you feel good about yourself.

There was a time when like many others, I wasn’t so nice to my skin.  As a child and teenager (when we are exposed to much of the sun damage that haunts us later in life) I spent most of my summers at the lake or outside riding horses or playing sports. I have two older sisters that I looked up to.  When I saw them putting baby oil on their skin to get the darkest tan, I followed right behind them.  I even followed them into the tanning salon.  I abused my skin, not knowing what kind of consequences it could cause later in life.  You know, dull, lifeless skin with pre-mature wrinkles, brown spots, broken blood vessels of the face, neck, chest and legs, sagging skin that makes us resemble our mother much earlier in life than we are ready for.   And let’s not forget…skin cancers.

Decades later, science has armed us with the knowledge of the kind of destruction sun exposure will cause.  We now know that most of the symptoms of aging skin (lines, wrinkles, broken blood vessels, brown spots, larger pores, rough texture, even freckles) are caused from sun damage not chronological aging.   We have had exciting advances in technology that allow us to now turn back time and correct some of sun damage we are marked with.  Thank you science!

So how did I change my once dull, lifeless, flaking skin into skin that makes complete strangers feel compelled to stop me just to tell me I have beautiful skin?  I did it with diligence and patience.  I give my skin a few minutes every morning and every night and in return it has given me back years.  So much so that most of our patients would tell you they are shocked to find out I have a daughter who is a Freshmen is College.

First and most importantly, protect your skin from the sun by using a medical-grade, physical sunblock such as Tisilc 60 and limit your sun exposure.  I’m not saying you can never see the light of day. Enjoy spending time outdoors, hiking, skiing, biking etc., just take advantage of opportunities to seek shade, wear a nice hat and re-apply your sunblock. Protect your skin every single day, not just in the summer, not just when it’s hot, but 365 days a year.  For those of you who are reading this and are thinking “but I have to get some color on my face”…try applying a bronzer instead.

Second, depending on the condition of your skin, you may need a skin rejuvenation treatment. A qualified Cosmetic Plastic Surgeon such as G.D. Castillo M.D. can help you decide which choice is right for you.  For me, it was CO2 Micro-Fractional Laser Resurfacing of the face. This procedure took my skin to a whole other level, erasing lines, pigmentation, restoring collagen and smoothing out the surface of my skin to a point that it feels much like the skin of a new born baby.

Third, MAINTAINANCE! Say it with me,” MAIN-TAIN-ANCE!”  This is where consistency and dedication come in.   This will require daily applications of medical-grade skin care products and possibly an occasional microdermabrasion, chemical peel or other facial treatment when needed.  Apply your medical grade skincare products consistently, every morning and every night.  Be sure to follow the regimen your medical skincare specialist and cosmetic plastic surgeon design for you.  You must be patient!  Quality, scientifically proven skincare products need time to make real changes in your skin.

This action is sure to set you for failure.  Your skin needs consistent applications of scientifically proven effective medical grade skincare.
