Mask Acne and Skin Irritations

Wearing a mask in public or when with others outside your household is essential in helping to prevent the spread of the Corona Virus.  However, wearing masks for extended periods of time can cause skin issues, so much so that there is a new trendy term to describe it:  “Maskne” (mask acne).

Sweating or breathing into a mask for an extended period can create a humid breeding ground for bacteria. This can result in cell break down and clogged pores which lead to black heads and acne breakouts, even for those who have lived a life of clear skin.  Friction from the mask that is too loose or does not fit properly can cause a breakdown of the skin’s protective barrier, resulting in skin rashes, breakouts and skin irritations.

What Can I Do To Prevent Mask Acne or Skin Irritations?

  1. Avoid using harsh or drying soap.  Certain cleansers can strip protective oils from your skin which are necessary to keep your protective barrier in-tact. If your skin feels dry or tight after washing, it is not the right cleanser for you. Using a harsh product can make your skin even more sensitive to mask friction.
  2. Wash your mask regularly! Re-using a mask that has make-up, debris or skin oils on it can create the perfect breakout environment.  Silk masks and some high-grade cottons are typically more gentle on the skin.
  3. Take off your mask when you are not with others. Put it in the sun (like on the dash of your car while driving…) The UV rays will help to kill the bacteria until you can wash it that evening. This is perfect for times when you have been sweating during a work-out, or exposed to summer heat! Let the sun zap some of the bacteria for you!
  4. Choose masks that fit properly!!! Masks that are too big will rub and cause friction, not to mention that loose masks DO NOT give proper protection to you or those around you. Your mask should fit snuggly around all the edges of mask.
  5. Apply a moisturizer. Moisturizers with ceramides like Replenishing Cream or Revive Recovery Complex from Dr. Castillo’s skincare line can help form a protective layer between your skin and mask and reduce irritations since ceramides are lipids that form the skin’s protective barrier. Products like Ultra-Hydration Booster (hyaluronic acid) can offer some soothing hydration to more intense acne breakouts to reduce irritation.  Replenishing Cream also helps to reduce sebum and provides anti-bacterial properties to reduce break-outs.
  6. Use a physical sunscreen formulated with zinc oxide. Zinc Oxide has anti-inflammatory properties that can both soothe and heal and will provide a buffer between your skin and areas where your mask rubs like the bridge of your nose, behind your ears, your cheeks, chin and jawline.  Avoid chemical sunscreens as the chemicals can create irritations and burning sensations.
  7. Let your moisturizer and sunscreen dry completely in between layers and before putting on your mask.
  8. Apply Clarifying Mint Mask (available at Dr. Castillo’s office) 1-2 times per week. This mask is formulated to purify, deep-clean, gently remove dead cells and minimize pore size, all while delivering a much-needed dose of moisture.
  9. Be careful with “home-spa” chemical peels and exfoliants. Aggressive exfoliation can compromise the skin’s barrier making it easy to develop or increase acne breakouts and irritations.
  10. Seek medical professional skincare advice / treatments to control your skin concerns during this time if at-home care does not resolve your skin issues.

POST-SURGICAL PAIN PUMPS

A common reasons patients put off having the cosmetic surgery they have always dreams of is their concern about post-surgical pain. While pain is a valid concern, it doesn’t have to be. Today, we offer our patients a simple way to manage pain without pills (narcotics).

This technology is called a pain pump. While the majority of the surgeries we perform at Cosmetic Plastic Surgery do not results in much post-surgical pain, utilization of a pain pump can offer a more comfortable recovery period for those procedures that do or for those individuals who have a low pain threshold.

A pain pump is a simple, non-electrical device that delivers local anesthetic (numbing

medication) such as Marcaine, directly to the surgical site after your surgery, allowing you to recover from surgery without the typically discomfort.

The pain pump looks like a small ball or balloon and comes in it’s own strapped carrying case. Dr. Castillo connects the pain pump at the surgical site at the time of surgery. It will deliver medication to the surgical area slowly and continuously through tiny catheters for 3-4 days. As the patient, you do not have to do anything with the pain pump. It is automatically regulated; you just get to reap the rewards of steady relief!

The pain pump offers pain management without the side effects of traditional methods of pain control like oral pain medications. This means you can avoid the potential side effect that oral pain medications often cause such as:

      •mental fogginess             •drowsiness              •nausea             •vomiting             •dizziness            •constipation

For patients who are narcotic sensitive or allergic, this can be a first-rate alternative for pain management. Patients who elect to use pain pumps are typically moving, driving and functioning and return to normal life activities more quickly since the affects are limited to the surgical area, the rest of your body & mind remain unaffected and have normal function.

While it does add about $350.00 to the cost of your surgery, the feedback we receive from our patients is that use of the pain pump provided them with a much smoother and quicker recovery than they had originally anticipated.

The Coca-Cola Tanning Trend

There is newer trend in tanning appearing on social media that involves putting “Coca-Cola” on the skin as a way to enhance a tan. Quite frankly, I cannot wrap my head around the thinking process or lack there of when it comes to this Cola-Cola” tanning trend. It is beyond comprehension. This is absurd on so many levels, unless of course the objective here is to find the fastest direct route to damaged, sun-burned, wrinkled skin with an increased risk of skin cancer.

First, take a look at this list of Coca-Cola ingredients directly from the Coca-Cola website:

Carbonated WaterHigh Fructose Corn SyrupCaramel Color,

 Phosphoric Acid, Natural FlavorsCaffeine

I guess the thought process here is to use the Carmel Color to stain the skin, giving the appearance of a tan. Believe me, there are definitely better, healthier options out there for achieving a fake bake look from body bronzers to spray tans. Check out my last two blog entries for my latest experience and lessons learned with airbrush spray tanning.

The Phosphoric Acid alone is enough to stamp “recipe for disaster” all over this one! Phosphoric Acid is a corrosive. This means it can remove the enamel off your tooth or polish old metals and coins. And… people want to put this on their skin? Nooo thank you!   Remember even simple exfoliation increases your risk of burning and sun damage when exposed to UV radiation.

Second, Thirty years ago we didn’t have the scientific support to enlighten us about the dangers of UV radiation. People used to apply baby oil and tanning oil and lay out on silver reflectors to get the deepest, darkest tan they could. But, the days of ignorant bliss are over! We now know that ultraviolet radiation from the sun and tanning beds is a HUMAN carcinogen! Say it with me…, “HUMAN carcinogen!” – That’s bad! Not even a tan can make that look good.

Third, if corrosive material and skin cancer isn’t deterrent enough, then for beauty’s sake, please! There are safe ways to get glowing skin! According the Skin Cancer Foundation, “an estimated 90% of skin aging is caused by sun exposure. “ Going outside unprotected is a sure fire way to develop an earlier onset of wrinkles, lines, rough texture and blotchy skin.

And Fourth, it’s gross! You will be a walking feeding frenzy for flies and other sugar seeking creatures like one of those sticky fly strips… a corroded, wrinkled, walking, sticky fly strip.

I rest my case.

Could a Non-Surgical Nose Job Really Work?

With the increased use of injectables through out the face, injectables as known as dermal fillers have come to play a role in managing small deformities of the nose.  Sometimes people refer to these derma filler injections to the nose as a non-surgical nose job.  In reality, it is not really a nose job, but rather the camouflaging of a deformity that can be corrected without the need of surgery.

A few examples of deformities that would possibly fall under this category would be:

1. A small dent on the side of the nose

2. A skin deformity related to scar and/or trauma

3.  A small nasal hump on a nose in which an experienced cosmetic or plastic surgeon could add dermal filler above and below the hump, adding volume to create the illusion of a straight nasal dorsum or straight nasal bridge.

While I do find patients who fall in this category in our consultations for nasal surgery in Champaign/Urbana, it is the minority of the noses that can be corrected through injectables.  Most of the patients seeking a nasal change do require changes about nasal cartilages and/or bones that would necessitate surgery as the treatment of choice.

When injectables can be used, one of two substances can usually be utilized.

1. Hyaluronic Acid– [Voluma, Restylane Silk, Restylane Lift, Juvaderm, Perlane, etc.,)

2. Hydroxy Appetite (such as Radiesse)

If you are thinking about undergoing nasal surgery and/or changes to your nose, your surgeon can easily point to the areas that can be corrected with either injectables and/or surgery and should be able to show you pictures of corrections that he has performed.  We have an extensive showcase of before and after photos of multiple types nasal deformities to demonstrate the corrective surgeries we have performed in our practice.  Our patients find this helps in a significant way when they are trying to address his own personal issues.

Lifestyle Lift Goes Bankrupt 

Many of us are familiar with the heavily advertised “Lifestyle Lift,” a so called “corporate facelift factory” that generated many headaches for many plastic surgeons having to treat or correct problems created by second-rate surgical procedures done in centers affiliated with Lifestyle Lift.

Unfortunately, in my Cosmetic Plastic Surgery practices in Savoy, I have seen numerous Lifestyle Lift patients with poor results who were seeking corrections of their Lifestyle Lift from an experienced Board-certified cosmetic plastic surgeon. This past March, the national chain filed for bankruptcy and the million-dollar per week advertising campaign that fueled their operation seized to exist.

The idea that hundreds of complaints have been filed over the course of their operation isn’t surprising.  The idea that you can pick a sophisticated surgical procedure that was described for over a hundred years and evolve it into a cut-rate, under-developed procedure and market it under your own corporate name sounds absurd but it is indeed what happened!  Imagine that you pick up colonoscopy and begin centers marketing “Lifestyle Colonoscopy” as something revolutionary, better, more special and then channel patient’s to your centers. Would we as physicians support that?

As physicians, we would not; because when we find a modification to a surgery that is an advancement for science, what we do is inform, publish and teach this modification or improvement to all the world for the benefit of everyone.  In the case of the “Lifestyle Lift,” it appears it was done for enrichment and financial gain of the corporate leadership, not medical advancement.

The sad part is that there was nothing new or better about the Lifestyle Lift.  It was just a marketing plot that deceptively hurt consumers.  Misinformed consumerism came at large price for many who were dooped by such an unethical marketing presentation.  The poor results generated from the lack of training, technique, patient safety practices, patient screening, and the lack of thorough pre & post-op care that sometimes occurred in these Lifestyle Lift affiliated centers resulted in a flood of complaints, complications and lawsuits against the “corporate facelift factory that can be found listed all over the internet  such as the list of complains at
