Caring for Winter Skin

Can you believe winter weather is just around the corner?  Where did the summer go? The leaves are turning.  Soon the trees will be stripped bare, left only with their grey, withered bark to show. The vibrant, supple flowers of summer, left unprotected, will become wilted and drab. Lack of hydration and sudden changes in temperature can have this kind of effect on your skin if you do not take the proper measures to protect it.

As the seasons change, so do the needs of your skin. For most, colder weather can create dry, tight, flakey, blotchy, even irritated skin.  Let’s face it, nothing amplifies your little imperfections (lines, wrinkles, discoloration, aging) the way dehydrated winter skin can.

Think of your skin as your body’s “body guard.” The outer most layer of skin serves as a “barrier layer.” Its purpose is to protect the deeper layers.  This protective barrier locks in the needed hydration and skin nutrients to maintain healthy skin. Because winter is a dry season, retaining moisture can be a big challenge. If your barrier layer becomes dry and depleted, it becomes much more difficult to lock in moisture.

So what can you do to protect your skin this winter?

  1. Keep your thermostat set at 68 degrees or less.  Higher temperatures will dehydrate your skin.
  2. Avoid taking long, hot showers.
  3. Increase your water consumption, especially if you are taking meds for colds, flu, antibiotics etc. Remember that coffee, tea, pop and alcohol can also dehydrate the skin.
  4. CHANGE YOUR HOME CARE ROUTINE WITH THE CHANGE OF THE SEASON:

Talk with a trained Medical skincare specialist, cosmetic plastic surgeon, or dermatologist about how to most effectively change your home skincare routine.

Here are a few general skincare tips…

CLEANSER:

You may need to change you cleanser during the colder months if your skin is affected by wind and climate changes. Pay attention to how your skin feels after you wash it (before you apply your moisturizer). Does it feel tight or dry?  If so, you may find it beneficial to switch from your “warmer weather cleanser” to a gentle, creamy, hydrating cleanser to avoid further irritation of your skin.  The purpose of warmer weather cleansers is to remove excess sebum (oil) and impurities from your skin. If your skin becomes irritated, removing too much sebum from your skin will create further dryness and irritation.  In addition, use a buff puff with your cleanser to help remove dead skin cells.

EXFOLIANTS:

Winter skin tends to look dull.  To keep it glowing, use a daily exfoliant such as a 10% glycolic acid.

HYDRATE / MOISTURIZE (in layers):

Maintaining healthy, balanced skin during colder months often requires an increased level of hydration. By this, I am not indicating that you should just apply twice as much moisturizer to your skin. Moisturizer building is best done in layers.  First apply a moisture-enhancing serum, such a hyaluronic acid, squalene or ceramides to enhance your protective barrier function.  Then apply your regular day or night moisturizer next.   Moisturizers that nourish the skin and replenish are essential for maintaining a healthy moisture barrier.

SUN PROTECTION:

I can’t say enough about this topic.  SUNBLOCK, SUNBLOCK, SUNBLOCK.  UVA rays are JUST as prevalent on bitter cold winter days. Remember that UVA rays penetrate deeper into your skin then it’s more well known partner- in- crime – the UVB ray. UVB rays are at least courteous enough to most skin types to let you know when you have been over-exposed to the sun (red or tan skin, burning, blisters…). UVA rays are a bit more sneaky. Because UVA rays penetrate more deeply, they do not leave the same immediate superficial signs of over exposure.  Eventually and unexpectedly,  it pops and says “surprise – got cha”, in the form of brown spots, broken vessels, leathery, wrinkled, yellowed, damaged skin.  Oh, and let’s not forget the more than one million new “surprises” it dished out this year alone in the form of “skin cancer.”

Keep in mind that sun reflects off snow, water, glass… further increasing its harmful abilities. I recommend using a sun block that has these blufour forms of defense.

  1. A physical block agent such as micronized titanium dioxide or zinc oxide at 7% or higher with an SPF of 45 or higher.
  2. A chemical sunscreen
  3. Anti-oxidants
  4. Tinted sun block – (iron oxides)

Only A Facelift Will Help You Now!

I think it is safe to estimate that at least once a week a patient, friend or family member asks what I know about the latest and greatest dream cream advertised as a “facelift  in a jar”  they’ve seen on tv, in a magazine, on a billboard or heard about from their mother-in-law.  So what I know about dream creams is that they are just that “a dream.”  I know I could say this in a more gentle way but truthfully, just look at the photos below of a facelift performed by G.D.Castillo, M.D. in his Champaign, Illinois cosmetic surgery practice.

If you are to the point in your aging process that you want or need a facelift; then have a facelift.  There are worse ways to spend your money, like on cream that is NOT a facelift.  Skincare products are NOT a replacement for a facelift.  They are accompaniments.  Now don’t get me wrong.  I am a big fan of medical-grade skincare.  It’s a necessity, right up there with air, food, Stilettos, and Botox.  It has not only done wonders for my skin personally but for hundreds of Dr. Castillo’s patients who are faithful in sticking to their regimen every day.

Medical-grade skincare products are part of daily at-home maintenance.  The use of effective daily skincare (I’m talking the good stuff – medical-grade, physician supervised, available only at your cosmetic or plastic surgeon office or a good dermatology office) along with occasional non-ablative skincare treatments when needed can help you buy some time in your aging process but once your there, well… your there.  Only a facelift will help you now.

I can completely understand why people would want to go gaga over the latest dream cream.  It would be ideal to avoid having surgery, or having to admit that you had surgery, to avoid the recovery process, the days off work that could be better spent sipping a cocktail under a palm tree somewhere. I get it.  However,  the reason to have surgery for most individuals, male and female, trumps the negatives every time.  I read somewhere that wrinkles are the road map of our lives, that they make us look distinguished, more worldly and wise.  I’m pretty sure they just make us look tired and old.  And if my face is my calling card, well that isn’t the message I want to send. “Hi, I’m Roxanne; tired and old, pleased to meet you.”

So what can a facelift accomplish, besides helping get your grove back?  In a nutshell, a facelift restores your youth.  It will remove your jowls and turkey neck, tighten your sagging muscle structure, lift the cheeks or mid-face and remove any loose, sagging skin. Seek only qualified, board-certified cosmetic or plastic surgeons.  Have more than one consultation.  And my best advice for today…when you are at your consultation, check out the staff.  Do they “look” like they have had work done because chances are they have. You can usually see an artistic theme… natural looking beauty, elegant, conservative, refreshed and well rested, glamorous or as our patients refer to it – “Hollywood”…over-plumped, over-stretched, over-done.  What is it that you are looking for? Observing a surgeons staff is one of the best ways I know to decide if your potential surgeon’s artistic views match your own.

THE PAPER TRAIL – INSURANCE AND MEDICAL NECESSITY

In Illinois Plastic Surgeon, G. D. Castillo, MD’s  practice, we do help patients whose cases qualify as medically necessary procedures; i.e., upper blepharoplasty https://www.cosmeticplasticsurgery.com/cosmetic-surgery/eyelid-surgery.cfm for vision constriction (excess skin obscuring vision),  or septoplasty https://www.cosmeticplasticsurgery.com/cosmetic-surgery/rhinoplasty.cfm  to correct a nasal obstruction that interferes with proper breathing and causes recurring sinus-type ailments  or breast reduction https://www.cosmeticplasticsurgery.com/cosmetic-surgery/breast-reduction.cfm to correct gigantomastia (overly-large breasts) resulting in postural changes causing neck and back pain, mastodynia (breast pain), dermatitis (skin irritation) and difficulty with movement.

When submitting a predetermination of medical benefits for these patients, insurance companies do ask for proof of medical necessity.  What does this mean for our patients?

What insurance companies are looking for is proof (a written medical record and/or diagnostic tests) that the patient will not obtain relief from their symptoms via conventional (non surgical) treatment such as medications, physical therapy and/or life-style changes.  Let’s take two hypothetical cases and examine the process and possible outcomes.

Patients Jane and Jenny Doe present for consultation with complaints of large breasts that are causing increasing discomfort .

Examination by Dr. Castillo discloses gigantomastia resulting in postural changes causing shoulder strapping (deep bra strap grooves from the weight of the breasts), neck pain, and back pain https://www.cosmeticplasticsurgery.com/patient-stories/.  They have difficulty with everyday movement and exercise.  They have painful, aching breasts (mastodynia) and suffer chronic dermatitis from breasts chafing against the chest wall.  Their body mass index (BMI) is within the normal range and both will require the insurance standard of 400+ grams of tissue to be removed from each breast.

Jane has suffered in silence for years.  She has not sought professional medical attention for her symptoms in the past and has treated her conditions (dermatitis, neck/back pain, etc.) with over-the-counter remedies – none of which have offered her the long-term relief she desires because they do not address the underlying source of her discomfort – gigantomastia.

Jenny, on the other hand, has been proactive in seeking professional assistance with the goal of long-term relief.  She has sought advice from her primary physician and has undergone treatment with both physical therapy (exercise to strengthen her musculature) and medications, all of which are documented in her medical history.  Upon determination that conventional treatment will not alleviate her problems, her physician has made a recommendation for breast reduction surgery and referred her for consultation with Dr. Castillo.

Our office submits a predetermination of medical benefits request to each of their respective insurance companies.  Jenny’s insurance company replies that the procedure of breast reduction surgery qualifies for benefits according to the limits/levels of her individual policy.  Jane’s insurance company replies that they require additional proof of medical necessity (unsatisfactory response to conventional therapy) and attaches a list of their requirements prior to breast reduction approval.

Both women have the exact same medical situation but Jenny’s efforts have resulted in a positive reply from her insurance company and she is able to schedule her surgery right away.  Jane is faced with the prospect of enduring her symptoms while she begins the medical documentation process or absorbing the financial responsibility alone.

I wish I could say that we’ve never experienced these scenarios, but we have.  It’s important to not ignore your medical symptoms and to seek professional advice from your physician.  The “paper trail” of medical necessity can be the difference between obtaining relief versus continuing on the spinning wheel of discomfort.  Arming yourself with the documentation you need for medical insurance coverage just makes good sense while suffering in silence is counter productive.

BALANCING BEAUTY

QUESTION:  When I read anti-aging articles, they often refer to facial balance.  Can you explain what that means?

ANSWER:  I belive what they are referring to is a “basic formula” used to describe a well-balanced face, proportionately speaking.  This balance is seen when the face’s prominent features, meaning the brows, eyes, cheeks and lips are proportionate to each other.   The eyebrows should be aligned with eachother with a slight arch toward the outer or lateral portion of the brow.  When looking at the eyes, there should be a 1/2″ between the upper lid and and the eyebrow.  The lateral or outer corner of the eye should angle up just a touch.  Prominent cheekbones are a sign of beauty.  They should be full and high set.  An oval shape that tapers itself back as you follow then back toward the ears is most desirable.  Last but not least are the lips.  The lower lip should be 2/3 larger than the upper lip.

Say Goodbye to Chest Lines, Wrinkles and Brown Spots!

I was speaking with one of Dr. Castillo’s breast augmentation patients during a skin rejuvenation consultation.  She explained to me that a few years back she had breast enhancement https://www.cosmeticplasticsurgery.com/cosmetic-surgery/breast-enlargement.cfm in Dr. Castillo’s Cosmetic Surgery facility in Savoy, Illinois and that she couldn’t be happier with her results and the entire Cosmetic Plastic Surgery experience.

However, she has some fine lines and wrinkles on her chest between her breasts along with a moderate amount of brown spots. She expressed that she would like to show off her new cleavage https://www.cosmeticplasticsurgery.com/photo-gallery/results.cfm?Category=10 without being embarrassed about having wrinkles between her new, perky breasts.  She asked if G.D. Castillo M.D. and Cosmetic Plastic Surgery Skin Restoration and Laser Institute offered skin treatments to remove the wrinkles and brown spots of the chest.

I explained to her that the vertical lines and brown spots she was describing are common among those who have been over-exposed to the sun. The sun breaks down elastin and collagen of the chest, creating lines, wrinkles, and sagging skin.  It also causes melanocytes (pigmented cells that create brown spots she is seeing on her chest) to over-populate the surface of the skin in attempt to protect itself from damaging UV radiation.

I explained to her that yes, we can help with her chest lines and brown spots.  To improve the look of her damaged skin, first, she would need to begin to protect her décolletage using medical grade broad-spectrum sunscreen 365 days per year.  UV radiation will create more severe damage to areas such as the chest, neck and hands. The characteristics of the skin of the chest, neck and hands simply cannot tolerate the same amount of UV radiation as the face. The skin of the chest is not as forgiving as skin surfaces such as the face when it is repeatedly over-exposed to the sun. The skin of the chest is thinner, containing less elastin / collagen.  As the elastin is broken down, lines, wrinkles and sagging of the skin WILL occur.

Unlike the face, skin of décolletage does not contain an abundance of oil glands.  Why is this a problem?  Having an abundance of oil glands is important because as oil glands cells divide, they can become almost any other type of epidermal cell.  Therefore, when cells get damaged, they provide new cells to help to restore your damaged skin.

The good news is there are a few ways to correct or at least improve the skin of your chest.

  1.  First, and most importantly, PROTECT your chest for the sun.  Use a medical-grade, broad-spectrum, physical sunscreen  every day of the year. I recommend one of two  physical sunscreens:  Sun Defense or Prime Defense By Castillo MD Skin Science.  Both have high percentages of physical block ingredients (Titanium Dioxide and / or Zinc Oxide) making them superior  choices for sun protection.
  2. Avoid tanning beds as UVA rays will only create more damage to your skin.
  3. Use UV protective clothing and a wide brimmed hat to provide additional protection.
  4. Consult G.D. Castillo M.D. or another Cosmetic Plastic Surgeon regarding the following treatment options:
    1. CO2 Micro-Fractional Laser Resurfacing – This will decrease the lines of your chest (décolletage) while building collagen within the treated skin.  It will also make a significant improvement to the texture of your skin and decrease or remove your brown spots or discoloration. More than one treatment may be necessary due to the delicate state of the chest skin.  Did you notice that I highlighted, underlined and italicized the “CO2” in CO2 Micro-Fractional Laser Resurfacing?  That is because CO2 is the key there!!     3. Medical Grade skincare products – There are effective medical grades skincare products created to specifically treat the chest area.  These products will not solve your problem alone but work well to support and prolong the effects of the above mentioned treatments. Medical grade means that they are of a higher strength and must be over seen by a physician therefor these are not your average over-the-counter disappointments.

HOW TO LOSE WEIGHT AND STAY THIN

A New Year is upon us.  Fort many of us it is a time to reflect on where we are today and to set new goals for the coming year.  Often those new goals include seeking a healthier lifestyle.

In both the Savoy and Bloomington locations Cosmetic Plastic Surgery Clinic, we receive a lot of questions about how to lose weight and keep it off from patients who are concerned about their increasing weight and those who want to maintain their results after procedures such as liposuction https://www.cosmeticplasticsurgery.com/photo-gallery/results.cfm?Category=5 or abdominoplasty https://www.cosmeticplasticsurgery.com/cosmetic-surgery/tummy-tuck.cfm .  What follows are the basics – that is, bringing a complex subject to a minimum denominator, we can say that what it takes to stay thin and fit is knowledge and a good lifestyle eating ethic.

How many times have you been around someone who tells you they hardly eat anything and that they gain weight, that they can reduce their caloric intake to nothing and that they still gain weight?  While many people would like for that to actually be the truth. The reality is quite different.  The very basic principle is that you will gain weight if you eat more calories than you burn – period, no exceptions.

Have you ever heard people say that if  you exercise enough you will lose weight?  While that is true, the basic principle here is that you absolutely cannot depend on exercise alone to bring your weight down.  It has got to be proportional with a decrease in the amount of food you eat, how that food is consumed and which foods you are consuming.

We will make things as simple as we can to understand what works.

1.     Water intake.  The basic principle of a good lifestyle eating ethic is to consume water.  Here, you must read exactly what it says – water.  Water is not pop, is not tea, is not anything else liquid.  Water is water.  Basic research shows that if you drink a minimum of one glass but better yet two glasses of water prior to your meal, you will consume less food and you will lose weight.  There are many reasons for this to happen, but suffice it to say that basic research has proven this without a doubt.  It is also important to drink water during the day.  In fact, often our bodies confuse hunger with dehydration.  If you have already eaten and are searching for a snack, replace that snack with a glass of water.  Wait 20 minutes.  You may find that your desire for a snack is gone.  Again, I must remark how important it is that it be water – not sweetened water, not flavored water, not anything else other than simple water.  Look around you and you will find that trim, thin, fit people drink only water.

2.     Reduce your portion sizes. Living in a nation where restaurants serve such large portions has been a big injustice to our health and over-all lifestyle.  Over the past few decades, we have begun to consume much larger portions than in the past.  As a result, we are becoming an obese nation with many weight related health problems. The more bad food we consume, the more our bodies crave them. The truth is, the older we get, the less caloric intake our body requires.  Simply put, we must consume less calories in our 40’s than we did in our 30’s, and less in our 50”s than in our 40”s, and less in our 60”s than we consumed in our 50’s and so on.  I suggest beginning to decrease you portion sizes by eating until you are no longer hungry instead of eating until you are full. Decreasing portion sizes can be difficult when you are enjoying your meal, however if you consistently use this rule, you may find it much easier to accomplish.

3.     Avoid salt, sugar, fat and preservatives. Remember that there are enemies in food that you must avoid.  Those are salt, sugar, fat and preservatives.  All of the above are contained in large quantities in fast foods, prepared foods, canned foods (even canned vegetables), packaged foods, etc. While busy schedules may make it difficult for some people to prepare their own meals and go back to a primarily natural source of foods, you must maintain a high alert state for foods that are high in salt, sugar, fat and preservatives.  Remove those enemies from your diet.  One way to avoid these types of foods, even with a busy schedule, is to prepare foods in advance.  Grill meats and fresh veggies in advance, refrigerate or freeze them until needed.  Cook enough brown rice for a few days. Consider adding a fresh veggie, or cilantro, pepper, or a small amount of white raisins and almonds to vary the flavor.  Retrieve it from the refrigerator as needed.  Prepare large quantities of low-sodium soups with fresh veggies, adding fish or chicken etc. if you choose.  Freeze it in portion size containers.  Not only will this method reduce the amount of food enemies in your diet, it will allow you to prepare healthy meals in no time.

4.     Avoid simple carbohydrates. Carbohydrates are not necessarily bad foods; however, simple carbohydrates create a high glycemic state; that is, they increase your blood sugar rapidly which is damaging to many tissues in your body.  It is important to avoid simple carbohydrates and that means sugar and refined grains (reduce to a minimum the amount of bread and pasta and make sure that when you eat these items they are prepared from whole grains).  Glycemic states not only create an easy avenue for your body to store fat, they are also highly damaging to tissues that contain a large number of blood vessels (for example, the retina).

5.     Vegetable proteins.  Increase the amount of vegetable proteins in your diet.  Vegetable proteins offer a great advantage as a food source. They burn slowly, which will keep you from becoming hungry.  You will go for longer periods of time with less hunger.  Vegetable proteins are contained in all legumes (foods that come from a pod) like any kind of bean, lentils, garbanzo (chick peas), etc.  Again you can easily prepare these vegetable proteins in advance and refrigerate or freeze them until needed.

6.     Exercise. Remember to maintain a program of exercise that fits your lifestyle.  That is, don’t create an exercise schedule that requires you to give more time than your schedule allows, it will set you up for failure. It should still allow for the normal routine of your life. That is; work, kids, food preparation, recreation, etc.  The amount of time spent exercising will vary from individual to individual, but it will require anywhere between 30-60 minutes three to five times per week.

7.     Honor the food that you eat. When you are ready to eat, do not do it on the go.  You cannot eat and drive a car.  You cannot eat and watch TV.  You cannot eat and study.  You cannot eat and etc.  That is, when you are ready to eat, drink 1-2 glasses of water then sit down, preferably with family and friends and eat your food slowly.  If you are spending less than five minutes eating your food – you are definitely eating too fast.  When you eat food in small bites it takes you longer to consume your food and you will give time for your body to register that food has been added to your system.  The mechanism of satiation (the feeling of being satisfied) takes some time to develop after food has been added to your stomach.  You also honor and value the food that you eat when you make it an event of importance.  It is one that deserves your full attention.
