WHAT IS THE FIRST STEP TO DISCOVERING IF A FACELIFT IS RIGHT FOR ME?

One might think the first step is to call your local facial plastic and reconstructive surgeon or cosmetic surgeon and schedule a consultation appointment. That’s not a bad option, but I recommend doing a little homework first.  By that, I am not suggesting to waste hours and hours online reading about facelift preparation, recovery etc., as much of the information out there is not correct or not cohesive with the instruction you will get with from your future surgeon.  That information is ultimately much better gotten straight from your actual surgeon.

Instead, decide how far you are willing to travel for your surgery. Consider that there will be several appointments involved over the course of this journey. Then research the Board-Certified Facial Plastic and Reconstructive Surgeons or Board-Certified Cosmetic Surgeons in that area. Look at their CV (curriculum vitae). What are their qualifications? What are their professional appointments (Example: Dr. Castillo is the Past President of the American Academy of Cosmetic Surgery…)? What type of experience do they have? How long have they been practicing? Look online at their before and after photos.  Do their before and after photos look natural? Do their photos look the way you envision looking after surgery? Once you feel comfortable with your research, schedule one or more consultations.

CHOOSING YOUR SURGEON

Choose a Board-Certified surgeon whom you feel comfortable with and one who has decades of experience! You want a surgeon who takes the time to listen and time to explain the risks, benefits, alternative treatment options, recovery and result expectations. Evaluate the staff as well.  How did they make you feel?  Were they warm, welcoming, knowledgeable, attentive, helpful? How long as Doctor’s clinical staff worked for him? Is there a lot of staff turn-over? Staff members tend to stick around longer if they are working for a skilled, trustworthy, genuine human being.

MAKING THE DECISION

Electing to have a facelift or any cosmetic procedure is a very personal decision. In the final analysis, remember that choosing to have a facelift impacts one person alone – and that person is you. It  should only be carried out if it is what you truly desire.

After your consultation, you may need time to digest all the information you gathered.  I would encourage you choose a facility that does not apply pressure or a timeline to your decision-making process.  Having a facelift can be an amazing experience.  Choosing the right (or wrong) surgeon, can be a life-changing one.

WHAT IS A FACELIFT AND WHAT CAN IT DO FOR ME?

Patients often consult with Dr. Castillo at Cosmetic Plastic Surgery Center when they begin to notice unwanted aging changes in the face and neck. Loss of collagen and elastin fibers, resulting in skin laxity, along with the loss of tissue and bone volume in the face are all normal parts of the aging process. When you combine those natural aging changes with gravity, years of cumulative sun damage, and hormonal changes, the result is often the formation of jowls, sagging facial and neck tissue, excess skin, wrinkles or skin folds, drier, thinner skin with pigmented spots and texture changes.

Funny how as a kid, you just can’t wait to “get older.”  You dream of the days when you can drive, go to bed when you want to, get up when you want to, make your own decisions, have total freedom… “BE AN ADULT.”  Well, apparently there is more to getting older than just gaining independence. Decades later, here we are “faced” with the full view of what giving up our youth really means.  Jowls. Sagging. Wrinkles.

WHAT IS A FACELIFT?

A facelift is a surgical procedure that tightens sagging tissue and skin of the lower face (cheeks, jowls, neck), resulting in a smoother, more youthful appearance.  A facelift is typically preformed under general anesthesia and will take on average 2.5 – 5 hours of surgical time.

During this procedure, excess tissue, skin, and fat are removed and muscle and skin that has been pulled downward by gravity over time is re-positioned to create a more rested, youthful appearance.

 

WHAT CAN A FACELIFT DO FOR ME?

In a nutshell, a facelift will make you look younger than you do today. It helps to repeal the forces of nature: time, stress, environmental factors, sun, gravity, hormonal changes, volume loss, decreased collagen and elastin production.

A facelift can:

  • Remove your jowls and turkey neck
  • Tighten your sagging muscle structure
  • Lift your cheeks
  • Remove any loose, sagging skin, therefore tightening the remaining skin
  • Give you a more defined jawline

WHAT TYPE OF RESULTS CAN I EXPECT?

Going into surgery with an appropriate set of REALISTIC expectations is the key to being happy with your surgical results.  Your surgeon can help you to understand what can and cannot be accomplished with a facelift. Here is where I would strongly recommend you listen to your surgeon, rather than Dr. Google (who by the way, didn’t actually go to medical school).  The results can vary from individual to individual depending on the severity of aging you present with before surgery, your skin quality, the amount of time you have spent in the sun over the course of your life, your genetics and your anatomy or facial shape and your lifestyle habits.

What Can I Do At Home To Reduce The Signs Of Aging?

More than ever, skincare has become a booming. progressive industry!  Finally, we have entered a culture  or state of mind that says it’s okay to value ourselves,  to invest in ourselves.   I’m not talking about entitlement,  That’s a whole other kind of topic. I’m talking about self-care and self-respect.  On one hand, that’s great.  We are living longer but we don’t necessarily want to look like it, am I right? This boom has created a push to develop and (hopefully) improve the very products you need to keep your skin looking good for as long as possible.  Now, on the other hand, wow – the sheer number of options out there is over-whelming, and it can be a disappointment and costly lesson to try to figure out which products work and which products are just marketing hype.  How many of you have drawers full of  product disappointment?

An at-home skincare regimen can mean so many different things to different people.  During each new skincare evaluation, I ask my patient to tell me what they are currently doing for their skin.  The answers range from an over-the-top laundry list of products to absolutely nothing.  I have always believed that delivering age-reversing results for our patients is best accomplished through dedication to communication, teamwork and most of all, consistency. You can start small with a smart, simple, medical-grade, proven-effective skincare product regimen at-home and move forward if needed, as you become more comfortable.

So, what can you do at home to reduce the signs of aging, sun damage and improve your skin’s texture and tone?  I will review a few of my favorite at-home product categories that will help you do just that!

Before I dive into discussing each basic product category, the first thing I want to talk about is the principal reason our skin ages.  Can you guess it? I’ll give you a hint… is NOT our age itself… but rather recurring sun exposure from birth onward. Studies indicate that approximately 80% of our lifetime sun exposure comes from “incidental” sun exposure, not a day at the beach.  Incidental sun exposure refers the UV rays you catch while walking your dog or in my case…walking my cat, walking to your car, sitting in front of a window in your home or while driving etc.  That being said, if you are not applying and re-applying quality, broad-spectrum UVA/UBA sunscreen on a daily basis, then the rest is for not because unprotected sun exposure will speed up your aging process and lines, wrinkles, rough texture, dull skin are certainly on the horizon.

Any basic anti-aging skincare regimen should include four application categories, although in some cases we may be able to streamline it even further.  Now of course, depending on the severity of your skin damage and your goals, we would customize a regimen specifically for you and skin concerns, but this will give you some good general knowledge.

 

The categories in review are:

  1. Cleansers
  2. Transformation products
  3. Anti-aging moisturizers
  4. Protection:

 

 

Okay, so the first product category I typically recommend to fight the signs of aging and achieve glowing skin is a good cleanser.

 

  1. CLEANSE:

In general, I prefer facial cleansers that contain exfoliating enzymes, AHA’s, BHA’s, and certain botanical ingredients. These ingredients will help to break down the build-up of dead skin cells, bacteria, grime, and sebum.  They can also stimulate circulation and improve cellular turn-over; all good things when you are looking for that natural glow.  Now do keep in mind, if your skin is really dry then you are better off using a gentle cleanser without AHA’s or other exfoliant properties until your skin’s protective barrier has been restored.

In the Castillo MD Skin Science Skincare line, I recommend one of the following cleaners depending on your skin type:

Advanced Enzyme Cleanser:

This cleanser can be used for dry, normal, oily, combo, acne sensitive skin types.  It is formulated to deliver a brighter, clearer complexion and encourages the healthy production of new skin cells. Papaya Enzymes and salicylic acid exfoliate dead cells, kill bacteria in pores and removes impurities and make-up without stripping the protective skin barrier.

 

Gentle Cleanser:

Gentle cleanser is for dry, normal, post-procedure, rosacea, sensitive skin types. It is formulated with botanical ingredients such as Clary Sage, Aloe, Cucumber Fruit, and Ginger Root. This cleanser will clean, condition and invigorate the skin, leaving it feeling soft, smooth and visibly younger looking without causing irritations. It is also a good choice for mature skin that is dry as aggressive cleansers can compromise the protective skin barrier even further.

 

  1. TRANSFORM:

This category refers to products that have a specific anti-aging job to do.  Transformation products might be used to reduce brown pigmentation, fight free-radicals, repair damaged DNA cells, brighten your complexion,  improve texture, reduce lines, wrinkles and large pores.  Depending on the skin concerns you may have, one of the following products might be recommended in your anti-aging at-home treatment plan.

 

Clear Skin Pads

These pre-soaked pads are a quick and simple way to deliver an at-home, anti-aging micro-peel to exfoliate and improve dull skin, rough texture and lines or clean out pores, correct acne and prevent future breakouts.  Clear Skin Pads soften and sooth acne-prone, inflamed skin while reducing irritations and redness with a botanically infused formula.  Contains Glycolic Acid, Salicylic Acid, Lactic Acid, and Yeast, Horse Chestnut, Ivy, Burdock, Olive, and Clary Sage Extracts.

 

Tretinol PM Serum

Beyond sunscreen, topical Vitamin A products (retinoids) such as Tretinol PM Serum are the most beneficial, proven-effective anti-aging topicals available.  Tretinol PM Serum stimulates cellular regeneration which improves tone, texture and hyperpigmentation. It reduces lines and wrinkles (who doesn’t love that)!  It is naturally derived, sulfate and paraben-free and contains antioxidants.  This is an after dark product only.  You should not apply this product during the daytime as it can make you more sun sensitive and could increase your chances of developing unwanted pigmentation.  If you are going to use this product you should always wear a UVA/UVB broad-spectrum sunscreen during the day.  Vitamin A products generally have a reputation for being harsh at first.  However, Tretinol PM Serum by Castillo MD Skin Science is impressively gentle due to the product’s silky emollient delivery system.

 

Antioxidant Serum:

This is a powerful combination of medical-grade Vitamin C (15% L-ascorbic acid), Vitamin E, Ferulic Acid and a peptide complex. It enhances collagen syntheses, which means you can expect firmer more supple skin.  Vitamin C is the ultimate skin brightener. Not only will it brighten dull, lifeless skin, but this antioxidant serum inhibits melanin production which helps reduce unwanted pigmentation and melasma,  leaving your skin clearer and luminous.  Antioxidant Serum neutralizes free radicals to protect the skin from cellular damage, resulting in healthier, more radiant skin inside and out! It even increases the SPF in sunscreen as it helps to fight UVA/UVB sun damage.  Vitamin C reduces sunburns, redness and irritation. It is a wonderful anti-inflammatory and increases your skin’s ability to produce collagen. This product can be used twice daily. It is formulated for dry, normal, oily, combo, acne sensitive skin types.  Note:  Vitamin C products are known to very unstable and in those cases it’s not likely to deliver much benefit.  It is important to choose a Vitamin C product formulated with  both Vitamin E and Ferulic Acid.  These two ingredients help keep the Vitamin C stable so you can enjoy the benefits.

Pigment Reducer:

This is a recent addition to Dr. Castillo’s skincare line.  This is another “after dark” only product.  It is an advanced, multi-active formula designed for rapid and significant improvement in the appearance of unwanted pigmentation. Pigment Reducer features an innovating lightening complex combined with alpha-hydroxy acids and botanical extracts to maximize penetration and stimulate cell turn-over.  This technology interrupts the melanogenesis pathway at multiple points, resulting in visible reduction in brown spots and other pigmentation.

 

Microdermabrasion Scrub:

Microdermabrasion is a wonderful, inexpensive addition to almost any skincare regimen in need of exfoliation or resurfacing to improve rough texture and allow your smooth, natural glow to show through!  It gives your skin an instant little boost of radiance.  It smooths dull, dry skin by exfoliating away dead cells and debris from your skin’ surface. It allows your skincare products to penetrate more effectively and will improve the look of your make-up.  While I am focused on faces to today, this product is also wonderful on your neck, chest, legs, feet, hands, lips, everywhere really! You will love how smooth your skin feels after using Castillo MD Skin Science Microdermabrasion Scrub.  This product is also beneficial in helping maintain the result of an in-office microdermabrasion in between treatments.

 

  1. MOISTURIZERS:

For many patients, winter means even drier skin. Not only does the outside air become drier (less humid) but when you turn on the heat, our homes and vehicles become drier too and our skin suffers.  Without proper hydration and moisturization, skin can become dull, dry, flaky and irritated. Lines and wrinkles will become more apparent and cosmetics will not perform as well if your skin becomes dry.  You can avoid losing that supple, youthful appearance by using a moisturizer that is appropriate for your skin type.

 

Lush Replenishing Cream:

Lush Replenishing Cream is a patient favorite for sure! It is an elegant moisturizer that combines 5% Niacinamide (Vitamin B3) with barrier enhancing lipids. It is intended for dry, normal, sensitive skin types and those with occasional minor acne breakouts.  It is formulated to decrease the appearance of sun damage, texture, hyperpigmentation and lines. Because of its anti-bacterial properties and oil reducing factors, those with mild-acne breakouts may also benefits from this moisturizer.

 

Calmative Cream:

Calmative Cream is intended for dry, normal, combo, oily and sensitive skin types. It is fortified with Vitamin K, Sunflower Oil, and Sweat Almond Oil. It is a concentrated, restorative treatment moisturizer that helps reduce dark under-eye circles and puffiness. It reduces the appearance of bruising, spider veins and broken capillaries. Calmative Cream is an excellent choice for those with conditions such as Rosacea who wish to reduce facial redness. Soothing, hydrating properties help skin retain its natural moisture. Calmative Cream combats symptoms of aging with antioxidant and anti-inflammatory properties to reduce the appearance of fine lines and wrinkles and calming emollients relieve redness and irritation associated with rosacea. It can be used for pre and post-treatment of fragile skin.

 

Revive Recovery Complex:

Revive Recovery Complex contains four different ceramides to restore the skin’s barrier function which helps the skin maintain its structure. Maintaining structure decreases water loss and increases factors which hydrate and protect the skin and aid in the skin’s recovery after exposure to stress. It is specially formulated with cactus and yeast extracts to significantly reduce irritation and redness caused by AHA’s, or  retinoids.  Appropriate for Dry, normal, combo, sensitive, and irritated, acne skin types.

 

  1. PROTECT:

The last step in this basic skincare routine as I mentioned earlier is the most important if your goal is to fend off the appearance of wrinkles and sun damage for as long as possible.

Sun Defense SPF 45

Sun Defense is designed to protect the skin from UVA / UBV and infrared radiation. It is a thin, lightweight, medical-grade, broad-spectrum sunscreen that is recommended for acne, oily, combo and sensitive skin types.  It absorbs quickly and contains high levels of Niacinamide along with Vitamin E, knotweed extract and hyaluronic acid to increase firmness and elasticity and reduce redness and blotchiness.

 

Physical Defense SPF 49

Physical Defense  is a very versatile, medical-grade, broad-spectrum UVA/UVB sunscreen. It is both gentle and light-weight.  It absorbs and dries quickly.  It offers our patients a more natural approach to sun protection by utilizing only chemical-free active ingredients.  Zinc Oxide & Titantium Dioxide sit on top of the skin to deflect UV radiation. Physical Defense SPF 49 creates a moisture barrier to stop the skin’s water from evaporating to help maintain hydration.  This sunscreen is beneficial for all skin types, especially those who are sensitive to over-the-counter chemical sunscreens. Barrier reef safe.

 

Prime Defense SPF 40:

If you love the feel of a good BB cream, you will love Prime Defense.  I always feel a little extra pampered when I am applying this product. It just feels so amazing!  Not only does it provide you with the necessary sun protection, but the pigments in this luxurious tinted sunscreen help to even out your skin tone.  Prime Defense contains protective anti-oxidants like Vitamin C & E, and Ferulic Acid Ester along with barrier protective lipids.  Prime Defense is a chemical-free sunscreen.  It even contains silicone polymers that provide a make-up primer base which leads to an even, smooth make-up application.

Pre & Post Microdermabrasion Treatment Instructions

MICRODERABRASION

To maximize your results and recovery, please adhere to the following Microdermabrasion directives: Post-treatment instructions will be reviewed with you and may be modified depending on your individual situation.

Pre-Microdermabrasion Treatment Instructions:

  • Please discontinue all Retin-A, retinoids, tretinols, Alpha-Hydroxy Acids, Beta-Hydroxy Acids, exfoliating creams, lotions or treatments, skin lighteners, or topical acne products 5-7 days prior to treatment.
  • Please keep skin well hydrated prior to treatment. If your skin is too dry, your appointment may be rescheduled.
  • Please avoid unprotected sun exposure prior to treatment. If you arrive with too much sun exposure (e.i. – sunburn), your treatment will need to be rescheduled.
  • Avoid waxing of the area to be treated for at least 2 weeks prior to treatment.
  • Please refrain from chemical peels, or other skin procedures for 2-4 weeks depending on the procedure.
  • Please inform us of any outside or at-home skin treatments prior to treatment or in-between treatments.
  • If you have an active cold sore or suspect you may be developing one, please reschedule your appointment. If you have a moderate history of cold sore, you may consider beginning a prophylactic dose of prescription Valtrex, 3 days prior to your treatment to help avoid a break-out.

 

Post-Microdermabrasion Treatment Instructions:

  • Skincare products such as antioxidants, moisturizers and sunscreen will be applied at the end of treatment so you can immediately apply make-up if you choose to.
  • You can begin to cleanse your skin the evening of your microdermabrasion treatment. Avoid using cleansers that are drying to your skin or contain AHA’s, BHA’s, abrasives or exfoliants.
  • Keep your skin well moisturized.
  • Apply and re-apply the medical-grade sunscreen recommended for you each day. You will be more sensitive to the sun for at least 14 days. Avoid direct or unprotected sun exposure.
  • Please refrain from using OTC or medical-grade retinols, tretinols, benzyl peroxide, topical acne products, AHA’s and BHA’s for 3-4 days post-treatment or as instructed at your treatment.
  • You can typically resume the use of topical prescription Vitamin A products such as Retin-A, or Renova, or topical prescription acne productions such as Differen, Clindamycin, Epiduo, Tazorac etc. within 7 days after treatment or as instructed at your treatment.
  • Please refrain from picking or scratching of the treated areas as it can cause unsuspected injury or irritation.
  • In the event that flaking occurs, keeping the area well moisturized will reduce the effect.

Monthly Microdermabrasion Club

Ways To Achieve Remarkable And Beautiful Skin Characteristics

While sitting in a small room in a Champaign office recently, I over-heard an elated young lady boasting about the beautiful diamond she received over the holiday. Her gushing excitement was infectious. I found myself feeling almost as excited as she, yet I couldn’t even see the diamond!

I then began to wonder “what it is that makes diamonds or other precious gems for that matter generate that type of admiration.” I would guess most of us regard diamonds as a thing of beauty… but why? What specifically is it that makes girls go gaga over diamonds? Is it the number of carats? Could it be the shape of the stone that is so enchanting?

To me, it is the clarity and color of the stone. I would much prefer a petite diamond with such superior color and clarity that it shimmers and dances in the light, and highlights every curve and contour, to a cloudy, three carat diamond that lacks life and luster. Would you agree? Now, don’t get me wrong. I am absolutely a fan of big rocks. I’m not trying to fool anyone here. Size does matter! It just falls short without superior color and clarity.

So why am I even talking about diamonds in a skincare blog?

Well, the characteristics that make a diamond sparkle and radiate beauty are very similar to the characteristics of beautiful skin. I mean is there anyone out there who would not be flattered to have their skin compared to a radiant, flawless diamond? Just as clarity and color are mandatory characteristics of a beautiful diamond, clarity and color are also key factors in creating beautiful skin.

Achieving superior skin color means that your skin is free of brown spots, freckles, redness, broken vessels and melasma. Skin clarity refers to skin that is free of rough texture, flakes, dull skin, large pores, acne, keratoses and moles. I would say that most people who are bothered by facial aging are focused on wrinkles. Be Proactive & Successful: Know How to Best Care for Winter Skin

Can you believe winter weather is just around the corner?  Where did the summer go? The leaves are turning.  Soon the trees will be stripped bare, left only with their grey, withered bark to show. The vibrant, supple flowers of summer, left unprotected, will become wilted and drab. Lack of hydration and sudden changes in temperature can have this kind of effect on your skin if you do not take the proper measures to protect it.

As the seasons change, so do the needs of your skin. For most, colder weather can create dry, tight, flakey, blotchy, even irritated skin.  Let’s face it, nothing amplifies your little imperfections (lines, wrinkles, discoloration, aging) the way dehydrated winter skin can.

Think of your skin as your body’s “body guard.” The outer most layer of skin serves as a “barrier layer.” Its purpose is to protect the deeper layers.  This protective barrier locks in the needed hydration and skin nutrients to maintain healthy skin. Because winter is a dry season, retaining moisture can be a big challenge. If your barrier layer becomes dry and depleted, it becomes much more difficult to lock in moisture.

So what can you do to protect your skin this winter?

  1. Keep your thermostat set at 68 degrees or less.  Higher temperatures will dehydrate your skin.
  2. Avoid taking long, hot showers, use warm water instead.
  3. Increase your water consumption, especially if you are taking meds for colds, flu, antibiotics etc. Remember that coffee, tea, pop and alcohol can also dehydrate the skin.
  4. Use a humidifier to decrease dryness in your home
  5. Avoid cranking up the heat in your vehicle.
  6. Changes your hydrators from serums, lotions or gels to hydrators with glycerin, lipids, ceramides, squalene… to reduce water loss in the skin.
  7. CHANGE YOUR HOME CARE ROUTINE WITH THE CHANGE OF THE SEASON.

Here are a few general skincare tips…

CLEANSER:

You may need to change you cleanser during the colder months if your skin is affected by wind and climate changes. Pay attention to how your skin feels after you wash it (before you apply your moisturizer). Does it feel tight or dry?  If so, you may find it beneficial to switch from your “warmer weather cleanser” to a gentle, creamy, hydrating cleanser to avoid further irritation of your skin.  The purpose of warmer weather cleansers is to remove excess sebum (oil) and impurities from your skin. If your skin becomes irritated, removing too much sebum from your skin will create further dryness and irritation.  In addition, use a buff puff with your cleanser to help remove dead skin cells.

EXFOLIANTS:

Winter skin tends to look dull.  To keep it glowing, switch from the more aggressive exfoliation methods you may have used in the summer months to gentler form of exfoliation or exfoliate less frequently.

HYDRATE / MOISTURIZE (in layers):

Maintaining healthy, balanced skin during colder months often requires an increased level of hydration. By this, I am not indicating that you should just apply twice as much moisturizer to your skin. Moisturizer building is best done in layers.  First apply a moisture-enhancing serum, such a hyaluronic acid to  bring more hydration to the skin, then follow with squalene or ceramides to enhance your protective barrier function.   Moisturizers that nourish the skin and replenish are essential for maintaining a healthy moisture barrier.

SUN PROTECTION:

I can’t say enough about this topic.  SUNSCREEN, SUNSCREEN, SUNSCREEN.  UVA rays are JUST as prevalent on bitter cold winter days. Remember that UVA rays penetrate deeper into your skin then it’s more well known partner- in- crime – the UVB ray. UVB rays are at least courteous enough to most skin types to let you know when you have been over-exposed to the sun (red or tan skin, burning, blisters…). UVA rays are a bit more sneaky. Because UVA rays penetrate more deeply, they do not leave the same immediate superficial signs of over exposure.  Eventually and unexpectedly,  it pops and says “surprise – got cha”, in the form of brown spots, broken vessels, leathery, wrinkled, yellowed, damaged skin.  Oh, and let’s not forget the more than one million new “surprises” it dished out this year alone in the form of “skin cancer.”

Keep in mind that sun reflects off snow, water, glass… further increasing its harmful abilities. I recommend using a sunscreen that has the following forms of defense:

  1. A physical block agent such as micronized titanium dioxide &/or zinc oxide at 7% or higher with an SPF of 40 or higher.
  2. Anti-oxidants
  3. Tinted sun block – (iron oxides)
