What Causes Wrinkles? Can Dermal Filler Really Help?

Most of us grow up understanding that wrinkles are just part of life – at least until it happens to us. Youth has this way of making us feel energetic and invincible, like we can conquer the world. That’s pretty hard to give up. It seems only natural that looking our best makes us feel our best, and feeling our best encourages us to keep conquering whatever comes our way.

Assuming you are one of the millions, NOT welcoming wrinkles with open arms, then you’re probably looking for a few good options for your aging arsenal.

There are many viable surgical and non-surgical wrinkle smoothing solutions that can have you back to daily life asap.  These include anything from Botox to laser resurfacing or a facelift. Sometimes dermal fillers such as Restylane Lift, Restylane Silk, Juvaderm or Radiesse can be a good solution to combat those are earlier stages of aging.

 

So what causes those pesky wrinkles anyway?

  1. As we get older, we experience a breakdown in collagen and elastin in our skin. We begin to notice appearance of fine lines, wrinkles and textural changes. You may begin to notice nasolabial folds or parentheses from the nose to the mouth, lip lines, marionette lines, sunken in lower lids and disappearing cheek prominence.
  1. As if reasons #1 & #2 weren’t enough of a blow to an aging ego, they are typically followed by the loss of boney structures and underlying tissue support. The loss of boney structure and tissue support creates more sagging and more wrinkling. As this occurs, skin moves downward with the help of gravity like that of a deflated balloon or wilting flower.

 

How can a dermal filler help improve unwanted facial aging?

Today, dermal fillers are not just used to give you a perfectly pouty lip, fillers can actually provide nice improvements to a mild to moderately aging face. They work by filling in lines and wrinkles and by providing much needed volume. Dermal fillers fill in lines around the mouth, or other areas of the face to create a smoother appearance. They plump up sunken in areas such as the lower eyelids, nasolabial folds and can restore or create a beautifully prominent cheek.

the signs of aging, they are often looking for a subtle change before they work their way up to a more dramatic change like facial surgery as the signs of aging progress. Some patients find dermal fillers to be an attractive choice because they can do it on a Friday and return to work on a Monday with no one being the wiser.

It is important to recognize that most people who are concerned about unwanted signs of aging will only benefit from fillers for a period of time. Fillers cannot take the place of having facial plastic surgery nor can they give you surgical results. They can only put a band aide on aging for a period of time. That being said, band aides are sometimes desirable. They can be a quick fix to mild lines and wrinkles, until the time is right for procedures like a facelift, a neck lift, or CO2 Laser Resurfacing.

Ask Our Expert: What Can a Non-Surgical Nose Job Really Accomplish?

With the increased use of injectables through out the face, injectables as known as dermal fillers have come to play a role in managing small deformities of the nose.  Sometimes people refer to these derma filler injections to the nose as a non-surgical nose job.  In reality, it is not really a nose job, but rather the camouflaging of a deformity that can be corrected without the need of surgery.

A few examples of deformities that would possibly fall under this category would be:

1. A small dent on the side of the nose

2. A skin deformity related to scar and/or trauma

3.  A small nasal hump on a nose in which an experienced cosmetic or plastic surgeon could add dermal filler above and below the hump, adding volume to create the illusion of a straight nasal dorsum or straight nasal bridge.

While I do find patients who fall in this category in our consultations for nasal surgery in Champaign/Urbana, it is the minority of the noses that can be corrected through injectables.  Most of the patients seeking a nasal change do require changes about nasal cartilages and/or bones that would necessitate surgery as the treatment of choice.

When injectables can be used, one of two substances can usually be utilized.

1. Hyaluronic Acid– [Voluma, Restylane Silk, Restylane Lift, Juvaderm, Perlane, etc.,)

2. Hydroxy Appetite (such as Radiesse)

Doctor Approved: How To Minimize Bruising After Dermal Filler Injections

Dermal fillers such as Voluma, Perlane, Juvaderm, Restylane or Radiesse are one of the most popular non-surgical solutions to improve aging skin. Dr. Castillo’s patients choose dermal fillers because it is a great way to increase volume in areas where the aging process has deflated their lips, cheeks, temples, or lower lids.  It is a quick way to fill in unwanted lines around the mouth and face to restore a more smooth, youthful appearance.

Though this treatment is not a facelift by any means, dermal fillers have made a place for themselves in the world of beauty enhancement and anti-aging.  Even though dermal fillers cannot provide the results of a facelift, they are at the height of their popularity today and their use is growing by leaps and bounds.

I think their increase in popularity has much to do with the idea of how they are marketed.  They are often referred to as a “no down-time” treatment in comparison to a facelift which does require a brief down time.You can have this treatment done on a free afternoon.  Dr. Castillo typically injects a local anesthetic to assure that you are comfortable during the procedure. You will be numb and have difficulty speaking for 2-4 hours after the treatment due to the localized numbing. Most people are ready to be seen socially by the next day.

While some patients can experience minor bruising or swelling, these side effects can be minimized or avoided with a little bit of preparation before their treatment.

     •  EXPEREINCE is Key!

Here is where preparation begins.  Do your homework.  Injecting dermal fillers within or under the skin is not as easy as it seems and far from a cookie cutter treatment.  You will be best served seeking a board certified Facial Plastic and Reconstructive Surgeon’s office.  They have the detailed training, expertise and experience to achieve your desired results.  Also keep in mind that on the rare chance that an adverse reaction could occur, you want to choose an injector who is qualified and experienced enough to recognize and treat your reaction. There is a multitude of decisions to be made and you want to make sure your injector is qualified to make them…

1.    Is this patient a good candidate for dermal filler?

2.    Which type of dermal filler should be used?

3.    How deep should the filler be placed?

4.    Which anatomical areas should be avoided?

5.    Which is injection technique is best or this case?

  • DISCONTINUE “BLOOD THINNERS” one week prior to your treatment.

Dr. Castillo recommends discontinuing all aspirin (acetylsalicylic acid), NSAIDS / blood thinners one week prior when possible, for his patients.  This will significantly reduce your chances for bruising. Blood thinners delay the coagulation process that would normally occur after an injection. This may result unwanted bruising. Over-the-counter medications such as Aspirin, Ibuprophen, Advil, Aleve, Motrin would fall under this category along with a number of vitamins and supplements such as fish oil, Vitamin E, St. John’s Wart and Primrose oil.  If you need to take mediation for discomfort, OTC Tylenol can be used.

  • AVOID STRENUOUS EXERCISE

Strenuous exercise can prolong your symptoms and is best avoided for the first 48 hours after your dermal filler.

  • APPLY A COLD COMPRESS

If swelling occurs, applying a cold compress to the treated area can help reduce swelling more quickly. It can help for the first 12 hours after treatment, but should not be used beyond 24 hours after your treatment.

Dermal fillers have become a very popular option in our Savoy (Champaign-Urbana area) office. Men and women love them because it instantly plumps, lifts and enhances their face and hands.
As we age, we begin to lose fat in the face, particularly in nasolabial folds (the lines that form from the nose to the mouth) and in our cheeks.  Collagen begins to break down as well. This creates lines and wrinkle. Skin begins to sag along with facial structures that leave us wondering who stole our youth in the middle of the night while we were innocently sleeping! Vertical lines develop around our mouth. Those once luscious, full lips become deflated. We are left wondering where we are supposed to apply our lipstick.
