You have packed away your flip flops and Daisy Dukes and pulled out your boots, long sweaters, leggings and warm winter coats; but did you remember change up your skincare routine?  This is the time of year when patients flood the skin rejuvenation suite in Dr. Castillo’s Savoy location, desperately looking for solutions to their concerns of suddenly seeing more wrinkles and sagging, dull, flaking, even red and irritated skin. 4982The unfavorable changes winter brings is often due to a drop in humidity, not only outdoors but indoors as well.  When we get exposed to the cold, windy, dry air, our first reaction is to rush inside and crank up the thermostat or jump a steaming hot shower. Unfortunately these “feel good” reactions de-hydrate our skin even more, causing our skin to look even more wilted.

Dry, winter weather can really pack a wallop.  Before you know it, dry air and harsh elements can heighten the appearance of wrinkles, and crepiness, flaking and loss of radiance.  Who wants to wake up with these changes right before the holiday gathering season?  Not me!  This calls for some simple changes in your everyday skincare routine to maintain your youthful, supple, radiant appearance.

I recently stayed in a lovely hotel for a few days in one of the more blustery weather states. I woke up after the second night feeling as if my face might actually crack in half.  True statement! My face felt dry, tight, rough, irritated and what’s worse… I looked as if I had aged 10 years over night!  NOT what I was hoping to get out of my vacation!  Well, I decided instantly that I was not going down without a fight! After all, I am a skincare specialist!

I immediately took some successful precautions to help return my skin to its normal condition.

  • I kept my hotel room temperature at 67 degrees – higher temperatures will cause dry skin.
  • Avoided hot showers that feel so good in cold weather – again causes dry skin.
  • Added a humidifier to add moisture to the room
  • Increased my water intake
  • Loaded up on my skin barrier protecting moisturizer
  • Continued to apply my medical grade, broad-spectrum sunscreen. Remember that the combination of winter sun along with snow glare can really damage your skin.

Wrinkled Or Not, Here I Come

Wrinkled was never  what I dreamed of becoming when I grew up; a zoo keeper, a talk show host or entrepreneur,  sure, but I checked carefully and wrinkled has NEVER appeared on my list, not even once!

We had some new staff photos taken recently at Cosmetic Plastic Surgery Center.  When we got them back I was startled to see a slightly older version of me in the photos.  I was forced to acknowledge that I too am slowly creeping up on a time in life when minor changes in my facial structure are beginning to occur.  When I look closely in the mirror, I can now see a hint of my mom looking back at me.  I know there will come a time when decisions will have to be made.  Will I decide to age gracefully or choose to defy it with a facelift.  Let’s see.  I better check my list one more time just to be sure.  Nope, still the word “wrinkled” does not appear.  Defy it is then.

How does one do that you ask?  Well it is easier if you have stayed out of sun, but for most of us that is not the case.  So that leaves protect and repair.  Both my parents were diagnoses with skin cancer while I was in my mid to late twenties, so really just a few years ago. ; -)  Hey… there is no judgment here ok.  This is my blog. Besides, like I always say “if age is just a number than I would prefer to keep mine unlisted.”

Watching the anguish my mother endured when the tip of her nose had to be removed, leaving it disfigured and later when my dad who had a large cancerous lesion in the center of his forehead, I realized the true dangers of the sun. Protection is a must when abstinence isn’t possible.  (from the sun!)

At that point, I began a daily regimen of sunscreen and swore off tanning.  That alone has bought me some time.  I am faithful to my medical skincare regimen.  I don’t jump around trying the latest and greatest infomercial product.  That’s a tough one for most of us – advertising can be so convincing.  I stick with medical grade skincare products only, never anything over-the-counter.  That’s not to say that there aren’t good OTC products out there, it is just that I value my time and money more than that and refuse to spend time and money hunting for something that “might” do the trick when I can go straight to medical grade skincare and feel confident that my investment will pay off.

I was recently telling a girlfriend that I am considering having full face laser resurfacing in the fall.  She responded “For what?  You don’t have a wrinkle on your face.”  Which of course isn’t quite accurate and to which I responded “yes, and I’d like to keep it that way.”  I explained to her that I am a firm believer in preventative (early) maintenance for multiple reasons.  First, addressing aging earlier rather than later gives me the opportunity to enjoy my results for as long as possible.  Why wait until “I am bad enough or old enough, or wrinkly enough to justify making improvements.” The only thing that will accomplish is that my self-esteem will take a hit.  I have always believed in taking care of myself with exercise, healthy food choices, and a clean lifestyle.  I want to enjoy the rewards of my efforts.  What if you just found out your home had a few termites, would you wait until the foundation was ready to collapse before something about it? Of course not, so why should our bodies be any different?

I believe early maintenance is beneficial because I think it provides more privacy.  The more there is to repair, the more dramatic the change can be.  Friends and family are less likely to suspect cosmetic surgery and more likely to assume that I am more well rested or that my new hair cut just took 10 years off my appearance.   Well… unless you write a cosmetic surgery and skin rejuvenation blog and tell the whole world about your anti-aging game plan… that does tend to cause some suspicion.

 

So will I have a facelift?  Well if the little things I do along the way; medical grade skincare, exfoliation treatments, Botox Cosmetic, laser resurfacing aren’t enough to keep the peace within myself, then, yes, I would consider a facelift when the time is right.  I would choose a traditional facelift method.  I am not interested in entertaining the idea of something like the Lifestyle Lift.  I have witnessed too many patients who have come to our office because they had the Lifestyle Lift somewhere else and it didn’t turn out as they hoped.  Then they need additional surgery (a proper facelift)  to correct it, something we never see with traditional facelift surgeries by Dr. Castillo.

If you’re still under the impression that Dog is man’s best friend, then you haven’t experienced Botox Cosmetic®. The Liquid Facelift – Truths In Reporting and Advertising?

On August 12, 2009, CBS had a program in which they demonstrated the so-called “liquid facelift.”  As it is shown, there were injections placed in appropriate areas of the face to diminish facial lines and furrows by injecting a product called Sculptra TM.  It incorrectly refers to Sculptra TM as being a  filler that was just recently approved, since it has been approved by the FDA for years for the treatment of immune-suppressed patients, and particularly directed at AIDS patients.
I take issue with describing this injectable as a “liquid facelift.”  In fact, it is not.  It is simply an injection to replace some of the volume loss in certain areas of the face.  It definitely does not give or even approximate the results of a mini facelift or regular facelift.  To compare the results of an injectable to a facelift is grossly unfair to the public and plays on the sensitivities of those who are looking for an alternative to surgery.  If what an individual needs is a facelift, then they are wasting their money on temporary filler.
Please do not misunderstand me.  There is nothing wrong with injectables.  I use them in my practice frequently with great success.  The whole idea is that it is very important to match the problem with the correct solution.  By that I mean that if there are already set-in facial problems like jowls, sagginess and volume loss, the most effective way of dealing with it is through a surgical procedure. Anything else would be at best a compromise.  For most patients, it would be a waste and a disappointment.
If, on the other hand, the patient is looking to treat early aging changes such as volume loss without sagginess or jowls, commonly seen after age 30, then an injectable such as Sculptra TM would be a good alternative. It will help minimize some of the volume loss effects of aging for approximately a year or two.  However, the physician injecting this filler needs to have a very good artistic eye in order to be able to inject the filler where it will have the most effect.  The goal is to minimize the normal shadows of light and to minimize the highs and lows that tend to develop as a result of facial aging.
The cost is another factor one needs to consider since it is likely that one may need $1,000.00 and $2,000.00 worth of Sculptra TM to accomplish the desired effect.  This needs to be taken into consideration because when one uses facial filler the effects are only temporary.  Within a period of 6 to 24 months, the effect is completely lost. When comparing this with the results of a surgical procedure like a facelift, one must realize that though the effects of a facelift may be modified by time (the aging process), the improvements will be with the person for life.
The report stated that you could go out the night of the injection and that is in essence, untrue.  Even in the hands of the most skilled professional, a moderate amount of swelling occurs with the injection of any filler within the face.  Bruising can also occur.  One should probably allow 3-4 days after an injectable before planning to attend a social engagement.
As with any other filler, Sculptra TM is a volume replacement agent which will be useful on a temporary basis. We should never call an injectable a “liquid facelift.”  We should call it exactly what is it, which is the correction of volume loss by use of a facial filler (injectable). This is much more accurate and honest reporting.
