This 63-year old from the Bloomington/Normal, Illinois, area was seen for abdominoplasty (tummy-tuck). She was dissatisfied with her midriff. At age 63, she kept in good shape with daily exercise; however, she noticed that since the birth of five children, her midriff had changed with an inability to loose the fat and volume, which was arranged in two belts - one higher than her umbilicus and the other as an apron below the umbilicus. With a weight of 150 pounds and at 5'2", she found it difficult to wear clothing because her tendency to being built like an apple was made more apparent following the steady accumulation of fatty tissue that happened with the birth of five children. She was seen in consultation for possible abdominoplasty.
During our consultation, I indicated that a large part of her problem was not only related to her abdomen, but also due to the accumulation of fat above the buttock region. This fat continued with wide hips and a poorly defined, high waist. I indicated that an abdominoplasty would not be able to resolve those problems and that I felt that liposculpture would be a better procedure. She was advised that liposculpture would not remove the redundant skin from her abdomen and that she would likely end up with some looseness of skin in and around her abdominal area. She indicated that she was more interested in how she looked clothed than how she looked in the nude, and that she would be agreeable with the possibility of having to deal with loose skin following liposculpture and the possible need of a secondary abdominoplasty.
Liposculpture in Champaign/Urbana’s Cosmetic Plastic Surgery fully accredited outpatient surgical facility was carried out with liposculpture of the upper and lower abdomen, waist, hips, back, upper buttocks and buttocks. Removal of a little more than 5,100cc of pure fat was accomplished in what we refer to as a combination volume reduction, body-contouring liposculpture. Small stab entry points were carried out where narrow 2mm cannulas were used. With the aid of ultrasonic energy, fat was melted and sculpted out of these areas. She was fitted with a post-operative garment that gave concentric support to the operated areas, and allowed to resume light activities immediately following surgery. Other than black and blues and moderate swelling of the operated areas, her recovery was easy and she resumed her normal house chores within 2 days of the surgery. She went on to resume moderate exercise within 7 days of the surgical procedure, and unrestricted exercise within 3 weeks.
She was last evaluated post-operatively at 3 months where she stated that she was delighted with her results. She viewed her body as “better than ever” with better proportions than at any other time of her life. She was truly bubbly and exuberant about her results. When I pointed to the mild wrinkling of skin below the navel, she stated that it was getting better and it did not bother her.
In viewing pre and post-operative photographs, one can appreciate a much narrower waist, which is now located anatomically just above the hip bones. She has normal projection of the gluteus in her rear musculature with round projecting buttocks, which flow into narrower hips. Her abdomen has lost the two wide fat bands that characterized it before with a normal contour of a tight abdominal musculature.
This is an example of how powerful a tool liposuction can be when properly applied to the correction of uneven deposits of fat.
