This 31-year old from the Bloomington/Normal area was seen with a chief complaint of droopy breasts with loss of volume. She is 5'6 and 125-pounds, and noticed that after her two pregnancies the slight breast droopiness that she had always had became much worse with loss of volume and deflation of both breasts. Her pre-operative photographs show a very significant droopiness with the nipple areolar complex being much below the normal position of the mid level of the arm region. Both nipple areolar complexes point downward, with an asymmetry of the breast with the left breast being wider, bigger and lower than the right breast.

During her consultation, we agreed that she needed both a breast lift and an augmentation. Due to the significant degree of droopiness and asymmetry, the plan of action was that of doing a breast lift with a slight reduction of the left breast to equalize both breasts to match the right breast. We would then wait a period of four to six months to allow healing of the incisions and then proceed with the breast augmentation. In my experience, patients with a significant droopiness do better when the augmentation and lift are done as staggered procedures. The skin heals with less scar formation this way because there is less tension in the skin lines. She agreed with the plan as discussed in consultation. The day of surgery, the patient arrived to our fully accredited outpatient surgical facility in Champaign-Urbana for a bilateral breast lift (mastopexy) with a reduction of the left breast.

The post-operative pictures taken at 3 months show completely normal breast tear-drop shape and nipple areolar complexes placed at a normal mid-arm level. Her incisions are healing very well without scar formation; however, she has some hyperpigmentation of the skin below the nipple areolar complex, which will fade over a period of a few months. The breasts are now equal in size and while she is very happy with the result. Her wish is to proceed with a breast augmentation as we discussed in her initial consultation. This will give her more fullness in the upper portion of the breasts, creating a fuller breast with a size closer to the size she experienced during her pregnancies.

She and her husband were severely impacted by the changes in her breasts after her pregnancies. She has been very happy with the positive comments and support from her husband following her breast lift surgery.
